| Purpose Using the quantitative scale of gastric qi injury established by Professor Ru Qingjing to evaluate the situation of gastric qi injury after radical resection of esophageal carcinoma,and to explore the relationship between gastric qi injury and short-term prognosis after radical resection of esophageal carcinoma.So as to verify the scientificalness of the disease prognosis view of Chinese medicine "Get enough gastric qi will rapid disease rehabilitation" disease outlook.Methods In this prospective case-control study,60 patients after radical resection of esophageal carcinoma were randomly divided into observation group and control group,the observation group(30 cases)were treated with family enteral nutrition after discharge from hospital,control group(30 cases)were treated with regular oral feeding after discharge from hospital.The changes of gastric qi injury score,nutritional risk score(NRS2002),KPS score and QLQ-C30 score at discharge,1 month after discharge and 3 months after discharge were analyzed.The correlation between gastric qi injury score,NRS2002 score,KPS score and QLQ-C30 score was analyzed.Results 1.In both groups,the gastric qi injury score at 1 month and 3 months after discharge was significantly lower than that at discharge time(P < 0.01)and KPS score was significantly higher than that at discharge(P < 0.01).The improvement of gastric qi injury score and KPS score at 1 month and 3 months after discharge in the observation group were better than those at the time of discharge.The improvement of NRS2002 in the observation group was better than that in the control group at 3 months after discharge(P < 0.05).2.1 months after discharge,the score of the observation group on the QLQ-C30 function subscale was better than that of the control group(P < 0.05).In the symptom subscales,the scores of the scores of the two groups were lower than those in the control group(P < 0.05)except for the scores of pain,breath and economic difficulties.At 3 months after discharge,the scores of functional subscale and general health subscale in observation group were significantly higher than those in 1 month after discharge(P < 0.01),and the degree of improvement was significantly better than that in control group(P < 0.01).In the observation group,the score of the symptom subscale was lower than that of the first month after discharge,except that the score of economic difficulty was not statistically significant at 1 month after discharge.In the control group,the scores of general health status in the functional subscale were significantly higher than those in the first month after discharge(P < 0.05),except that the scores of emotional subscale were not significantly improved at 1 month after discharge(P < 0.05).In the control group,except nausea and vomiting,insomnia,loss of appetite and economic difficulty,the scores of the remaining sub-items were lower than those in the discharge for 1 month(P < 0.05).The scores of pain,shortness of breath and economic difficulties were not statistically significant between the two groups,and the other subitems in the observation group were better than those in the control group(P < 0.05).3.Research shows there was correlation between gastric qi injury scale and NRS2002 score,and the correlation was good.The functional subscale of the gastric qi injury scale and the QLQ-C30 scale,the subscale of general health condition are related,and the correlation degree is moderate.Most of the symptom subscales of the gastric gas injury scale and the QLQ-C30 scale are related except for the pain area and the economic difficulty area.Conclusion The application of family enteral nutrition after esophageal carcinoma surgery can promote the recovery of gastric qi injury,nutritional status and physical fitness,and can improve the quality of life of the patients to a certain extent,which is worth popularizing.The gastric qi injury score can reflect the short-term nutritional status and physical fitness of the patients after radical resection of esophageal carcinoma,and to some extent can reflect the quality of life of the patients."rise and fall of gastric qi" can be used to judge the short-term prognosis of esophageal carcinoma after radical operation. |
