Research On The Medication Regularity Of Traditional Chinese Medicine In The Treatment Of Idiopathic Membranous Nephropathy Based On Data Mining

Objective:By means of data mining,the clinical experience and medication rule of chief physician Gao Zhi in the treatment of idiopathic membranous nephropathy were summarized.Methods:The medical records of patients diagnosed with idiopathic membranous nephropathy in the Outpatient department of Nephrology,Wuhan Hospital of Traditional Chinese Medicine from October 2019 to December 2021 were collected.The patient’s gender,age,prescription medication and laboratory index of the patients were input into Excel software to establish a database and matrix.Then Excel software was used to count the frequency of drugs,screen out high-frequency drugs,and count the function classification,four qi,five flavors and frequency distribution of meridian return of high-frequency drugs.Cluster analysis,correlation rule analysis and network pharmacology analysis were used to summarize the core drugs and common drug pairs of chief physician Gao Zhi in the treatment of idiopathic membranous nephropathy,and to further summarize the rule of using Traditional Chinese medicine in the treatment of idiopathic membranous nephropathy.Results:1.A total of 128 drugs were involved in 120 prescriptio ns,51 of which were high-frequency drugs(frequency≥10 times).The top 15 drugs with higher frequency are Astragalus,Tuckahoe,Yam,Atractylodes,Dogwood,Dodder,Codonopsis,Salvia,CortexMoutan,Achyranthes,Licorice,Golden Cherry,Conioselinum univit tatum Turcz,Gorgon and Twig of Winged Euonymus.The main drugs a re tonifying drugs,followed by drugs for promoting blood circula tion and removing stasis,drugs for collecting astringency,drugs for clearing heat,and drugs for promoting water infiltration an d dampness.The drug properties of high frequency drugs are mainl y warm,followed by cold and flat properties;The medicine mainly tastes sweet,bitter and acrid,followed by sour,light and astr ingent,and mainly enters the liver,spleen,kidney,lung,heart and stomach channels.2.There are 17 core medicines in the high-frequency medicine,in cluding Astragalus,Codonopsis,Atractylodes,Yam,Tuckahoe,Dogw-ood,Dodder Licorice,Cortex Moutan,Achyranthes,Golden Cherry,Gorgon,Conioselinum univittatum Turcz,Salvia,Yizhiren,Twig o f Winged Euonymus and Barbed Skullcap Herb.There are 9 pairs of commonly used medicines in clinical practice,they are:Golden Cherry-Gorgon,Peach kernel-Safflower,Coix-Lysimachiae Herb a,JianQu-Perilla terrier,Dogwood-Cortex Moutan,HawthornJianQu,Yam-Dogwood,Smilacis Glabrae Rhizoma-Rhubarb,Prepared rhizome of Adhesive Rehmannia-Alisma Orientale.According to the analysis of association rules between drugs,AstragalusAtractylodes,Tuckahoe-Yam-Dogwood,Tuckahoe-Yam-Cortex M ou-tan,and Tuckahoe-Yam-Astragalus are the most commonly us ed drug pairing in clinic.Conclusions:1.Astragalus,Codonopsis,Atractylodes,Chinese yam,Poria,Cornus,Dodder,Licorice,Cortex Moutan,Achyranthes,Golden Cherry,Gorgon,Chuanxiong,Salvia,Yizhiren,Twig of Wing ed Euonymus and Barbed Skullcap Herb,a total of 17 herbs for Mr Gao’s core clinical commonly used drugs.These drugs are mainly based on Shenqi Dihuang Decoction,which is based on the addition and subtraction of prescriptions.They can nourish the spleen an d kidney,nourish qi and yin,promote blood circulation and remov e blood stasis,collect astringent and solid essence.2.The basic pathogenesis of idiopathic membranous nephropathy is the combination of deficiency and excess.The primary deficiency is mainly due to deficiency of the spleen and kidney,and is char acterized by blood stasis,damp-heat,dampness and turbidity.We should strengthen the righteousness and eliminate pathogenic fact ors,and treat both the symptoms and the symptoms.While invigora ting the spleen and kidney,we also can promote blood circulation and remove blood stasis,and simultaneously clear away heat and invigorate dampness,and invigorate the spleen and change dampnes s.3.The kidney is the zang of Yuan Yin and Yuan Yang,so the treatment drugs of idiopathic membranous nephropathy are mainly cold and warm,and the main thing is to balance Yin and Yang.4.Strengthen and protect the spleen and stomach can increase the source of qi and blood biochemistry,At the same time,we can no urish the kidney,so as to achieve the next day to nourish congen ital,congenital warm and nourishing the day after tomorrow,impl ies the meaning of "spleen and kidney with tonic".5.Blood stasis is often accompanied by the whole course of idiop athic membranous nephropathy,which is one of the pathological fa ctors leading to the continuous development of idiopathic membran ous nephropathy and the decline of renal function.If blood stasis is not removed,it is difficult to recover kidney qi.Therefore,the use of blood-activating and stasis-removing drugs should be throughout the treatment of idiopathic membranous nephropathy.
