Clinical Characteristics Of Latent Autoimmune Diabetes And Type 2 Diabetes Mellitus In Adults

Objective The clinical characteristics and chronic complications of latent autoimmune diabetes in adults（LADA）and type 2 diabetes mellitus（T2DM）were compared to provide evidence for early and reasonable intervention of LADA patients.Methods The general data,such as gender,course of disease,BMI,comorbidities,complications,as well as blood glucose,serum C-peptide,blood lipids,renal function,Liver function and other items of LADA patients and T2DM patients admitted to Yanbian Second People’s Hospital from January 2017 to December 2019were collected to compare the differences and clinical characteristics between the groups.Influence factors of two kinds of diabetic C-peptide and glomerular filtration rate（GFR）were also analyzed.Results1)The levels of BMI,C-peptide,DBP,TG,ALT,GGT,BUN,CREA and SUA in LADA patients were lower than those of T2DM,while the levels of m Alb,UACR and GFR were higher than those of T2DM,with a statistically significant difference.2)The detection rates of MS and high TG in LADA patients were lower than those in T2DM patients,and the differences were statistically significant.3)In LADA patients with low GAD level,C-peptide was positively correlated with BMI,TG,AST,ALT,GGT,CREA,SUA and UCR,while negatively correlated with the HDL level.In high GAD level,C-peptide was positively correlated with BMI and SUA,while negatively correlated with the HDL level.4)The influencing factors of C-peptide level in LADA patients were BMI,HDL,GGT and Ca²⁺.The influencing factors of C-peptide level in T2DM patients were BMI,TG,ALT and HDL.5)The influencing factors of GFR level in LADA patients were Hb A1c,K⁺,BMI and age,while the influencing factors of low GFR level were Hb A1c and K⁺.The influencing factors of GFR level in T2DM patients were Cys-c,BUN,age,SUA and BMI,while the influencing factor of low GFR level was Cys-c.Conclusion1)Compared with T2DM patients,LADA patients had lower BMI level,worse islet function,and lower incidence of high TG and MS.2)The influencing factors of C peptide in patients with LADA and T2DM were different.It suggested that the isletβcell function of LADA patients and T2DM patients should be evaluated according to different influencing factors.3)The influencing factors of low GFR in patients with LADA and T2DM were different.It suggested that diabetic kidney disease in LADA patients and T2DM patients should be evaluated according to different influencing factors.
