| After entering the 21 st century,with the social development and the improvement of people’s living pressure,psychological problems have become increasingly prominent.More people need psychological counseling and mental health treatment,and the state’s attention to mental health has gradually increased.The "healthy China" strategy proposed by the 19 th National Congress of the Communist Party of China emphasizes the health services for key groups,including psychiatric patients,which has ushered in unprecedented new opportunities for the development of mental health specialized hospitals.However,public psychiatric hospitals are also facing the dual competition of general hospitals with stronger comprehensive strength and more flexible non-public psychiatric hospitals.On the one hand,compared with private specialized hospitals,the salary system of public specialized hospitals is more constrained and inflexible;On the other hand,due to the relatively single department and small volume,compared with public general hospitals,the comprehensive strength of public specialized hospitals is relatively weak and less attractive to talents;In addition,due to the public welfare of public hospitals and the medical reform goal of solving the problem of "expensive medical treatment",public mental health hospitals are also faced with the contradiction between ensuring the competitive performance of medical staff and reducing the medical expenses of patients.Public psychiatric hospitals can alleviate the above contradictions by improving the performance appraisal system.There are some problems in the traditional hospital performance appraisal system,which often links personal income with hospital income,and the distribution among medical personnel is unreasonable.In order to meet the challenges brought by the promotion of medical reform to hospital performance appraisal,hospitals need to build a performance appraisal system that can optimize the allocation of resources,so as to improve the work efficiency of medical personnel and the quality of medical services.However,there are still many problems in the domestic research on hospital performance management,such as a small number and limited quality of papers,so there is still a large research space.Under the background of the gradual deepening of medical reform,in order to solve the problems and challenges faced by the performance appraisal reform of public mental health hospitals,this paper introduces the resource-based relative value scale(RBRVS).This performance appraisal tool evaluates the workload of medical staff based on the labor value of medical projects.Under the new medical reform,performance reflects labor value and optimizes the allocation of hospital limited resources.At present,it has been introduced into China and achieved some results,but it is often used in general hospitals and rarely used in domestic mental health specialized hospitals.Therefore,this study aims to combine the strategic objectives and particularity of public psychiatric hospitals to build a performance appraisal system based on RBRVS.Firstly,it analyzes the impact of medical reform and health China policy on hospital performance appraisal,and explains the feasibility and necessity of establishing performance appraisal system based on RBRVS in public mental health hospitals.Secondly,it combs the process of constructing the performance appraisal system of mental health specialized hospital,and constructs the performance appraisal system suitable for Q hospital based on the theoretical basis of RBRVS and the specific situation of Q hospital.Finally,summarize the experience and lessons in the construction and implementation of the performance appraisal system of Q hospital,and put forward suggestions for other public hospitals to build a performance appraisal system based on RBRVS.The main research results of this paper are as follows: 1.Combined with theories and cases,this paper puts forward the construction process of hospital performance appraisal system based on RBRVS.2.Based on the theories of incentive theory and resource dependence,combined with the specific situation of the case hospital,this paper combs the process of constructing the performance appraisal system of specialized hospital based on RBRVS,including four stages of "preliminary preparation,index system construction,calculation and optimization and continuous improvement",so as to provide reference for the performance reform of the hospital;2.Based on the perspective of resource investment,Delphi expert scoring method is used to build a performance appraisal system for Q hospital.This paper introduces RBRVS to build a performance appraisal system for Q hospital from the perspective of resource investment.Based on the standard(2012),combined with the particularity of medical projects in mental health specialty hospitals,the labor value data of medical projects in mental health specialty hospitals are obtained by Delphi expert scoring method,which is used as the data basis for the construction of performance appraisal system;3.Combined with cases,the application and Implementation Suggestions of RBRVS are put forward,and its localization application is promoted.RBRVS is mostly used in general hospitals in China,and there are few application cases in specialized hospitals.Under the background of medical reform policy,combined with the case situation,this paper constructs a performance appraisal system based on RBRVS for specialized hospitals,summarizes its application and implementation suggestions,and also promotes the localized application of RBRVS in specialized hospitals. |
