Objective: Chronic prostatitis(CP)is a common disease of genitourinary system in clinic,with a high incidence and a large number of outpatients.Modern medical treatment of CP is mainly based on empirical medication,which lacks sufficient evidence-based medicine.Traditional Chinese medicine has certain advantages and potential in improving the clinical symptoms of CP,but its specific mechanism needs further discussion.At present,research shows that autoimmune factors play an important role in the pathogenesis of chronic prostatitis.IL-6 can induce the dimerization of gp130 by binding with its receptor IL-6R,and then regulate the expression of downstream cytokines and activate signal pathways,thus playing a variety of biological roles in cell proliferation and differentiation,immune regulation,inflammation regulation and so on.In the previous studies,Qianliexiao Decoction has achieved good results in treating CP,which can significantly improve NIH-CPSI score,TCM syndrome score and EPS-WBC number of patients,with a total effective rate,and can maintain normal immune response and block autoimmune inflammatory response by regulating the differentiation balance of Th17/Treg cells.However,IL-6 plays an important role in regulating the differentiation of Th17 cells.At present,the specific influence mechanism of Qianliexiao Decoction on IL-6 signal transduction process has not been clarified.This study intends to take the transmembrane signal transmission of IL-6 as the breakthrough point to clarify the regulatory effect of Qianliexiao Decoction on the expression of IL-6 and gp130 in EAP model mice,so as to comprehensively explain the mechanism of Qianliexiao Decoction in preventing and treating CP,and provide new ideas and experimental basis for the research of preventing and treating CP with traditional Chinese medicine.Methods: A mouse model of experimental autoimmune prostatitis(EAP)was established by injecting Freund’s complete adjuvant+purified solution of prostate protein.The pathological changes of mouse prostate were observed by light microscope,and the effects of Qianliexiao decoction on the expression of IL-6 and gp130 protein in EAP mice were detected by Western Blot.Results: 1.Observe the pathological changes of prostate tissue of mice in each group under light microscope: Normal group: the prostate tissue structure is normal,a small amount of secretions can be seen in the glandular cavity,the arrangement and distribution of glands are normal,the stroma is few,and no inflammatory cell infiltration is found.Group A: The structure of prostate tissue was disordered,the glandular cavity was irregularly changed or destroyed,accompanied by congestion and edema,fibrous tissue hyperplasia,and a large number of inflammatory cells infiltrated in stroma and gland.Positive control group: the prostate tissue structure basically returned to normal,some glandular cavities became smaller,and a few inflammatory cells infiltrated;Compared with the model group,the infiltration degree of inflammatory cells in stroma was significantly reduced.Qianliexiao low-dose group: the structure of prostate was disordered and destroyed,some glandular cavities became smaller,congestion and edema could be seen in interstitial tissue,and moderate inflammatory cell infiltration could be seen in interstitial tissue and gland;The degree of inflammatory cell infiltration was slightly reduced compared with the model group.Qianlie Xiaozhong dose group:most of the prostate structures returned to normal,some glands had secretions,no interstitial edema,and a small amount of inflammatory cells infiltrated in the interstitium;Compared with the model group,the infiltration degree of inflammatory cells in stroma was significantly reduced.Qianliexiao high-dose group: the structure of prostate basically returned to normal,without interstitial edema and no obvious infiltration of inflammatory cells in interstitial tissue;Compared with the model group,the infiltration degree of inflammatory cells in stroma was significantly reduced.2.Western Blot was used to detect the expression of IL-6 and gp130 protein in prostate tissue:Compared with the normal group,the expression of IL-6 and gp130 in prostate tissue of model group was significantly higher,and the difference was statistically significant(P < 0.05).Compared with the model group,the expression of IL-6 and gp130 in the high,middle and low dose groups of Qianliexiao decreased significantly(P < 0.05),while that in the positive control group decreased significantly(P < 0.05).Compared with the normal group,the expression of IL-6 and gp130 in the high,middle and low dose groups of Qianliexiao was significantly different(P < 0.05).Conclusion: 1.Qianliexiao Decoction can inhibit the inflammatory reaction of EAP mice’s prostate tissue,and make the prostate structure return to normal.2.Qianliexiao decoction can inhibit the expression of IL-6 and gp130 in the prostate of EAP mice,and the concentration of Qianliexiao decoction has obvious dose-effect relationship with the inhibition of IL-6 and gp130 expression.The mechanism of action may be to inhibit the transmembrane signal transduction of IL-6 by down-regulating the expression of IL-6 and gp130 and block its downstream signal pathway,thus inhibiting the autoimmune reaction of EAP model.
