| Background:In the middle-aged and elderly population,the incidence of osteoarthritis is no less than that of cardiovascular and cerebrovascular diseases,and it is often more severe.The most common type of osteoarthritis is knee osteoarthritis(KOA),and the most direct and effective treatment method is total knee arthroplasty(TKA).However,the problem of postoperative pain is a difficult problem that has plagued people for a long time.In today’s society where Western medicine analgesia dominates,floating acupuncture,as a new type of TCM acupuncture technique,has its own unique features in analgesia,but the scope of application and research depth need to be improved.Objective: To observe the perioperative analgesic effect of floating needles after unilateral total knee arthroplasty.Methods:A total of 60 standard patients who were hospitalized for knee osteoarthritis and underwent primary unilateral total knee arthroplasty in the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from December 2019 to October 2010 were selected and randomly divided into control group and experimental group.Group two groups,30 people in each group.The control group received conventional analgesia,that is,preoperative anesthesia was combined epidural anesthesia,and postoperative patient-controlled analgesia(PCA)with an internal volume of 100 ml,the main components of which were palonosetron,sufentanil,and Gaowu The dosage should be adjusted according to the patient’s gender,age,physical condition,etc.The standard pumping speed is 2ml/h,and each time the self-controlled pressing is 0.5ml.If the patient’s pain worsens during the connection period,the volume of the pumped liquid can be increased by pressing the self-controlled valve.The doctor needs to be informed and recorded.Pain pump.When the patient had strong unbearable pain,celecoxib capsule(Celebrex)0.2g was orally administered and recorded.If the pain is not relieved after 30 minutes of oral administration of Celebrex,intramuscular injection of parecoxib sodium(Titanic)40 mg or bunarizine hydrochloride injection(Jiantong Ding)50 mg is given,depending on the degree of pain of the patient.After the application of analgesic drugs,the name of the drug,the dosage and frequency of use should be recorded in time.On the basis of the control group,the analgesic regimen of the experimental group was based on the addition of floating needle analgesia during the perioperative period: two days before the operation,a total of one treatment;Treatment,1 treatment every other day,a total of 3 times.According to the group,the VAS pain score of each patient on admission(preoperative),6h after operation,18 h after operation,24 h after operation,48 h after operation,72 h after operation,96 h after operation,and 120 h after operation was recorded(under exercise state).The active range of motion(ROM score)of each patient was recorded at admission(before surgery),48 hours after surgery,72 hours after surgery,96 hours after surgery,and 120 hours after surgery.The types,doses,and times of analgesic drugs used by patients and whether adverse reactions occurred were recorded.All the data were sorted,the difference in clinical efficacy between the two groups was analyzed,and the analgesic effect of acupuncture in the perioperative period on TKA after TKA was observed and determined.Results:There were no significant differences in gender,age and preoperative data between the two groups.In terms of exercise VAS score,there was no significant difference between the two groups at the time points from6 h before operation and 48 h after operation to 120 h after operation;there were differences in the data at 18 h and 24 h after operation,and the data in the experimental group was better than that in the control group.There was no significant difference between the two groups in terms of active ROM score of the affected knee joint at each time period,postoperative use of various analgesic drugs,and adverse reactions.Conclusion:Comparing the data of the two groups of patients,after statistical analysis,it is concluded that floating acupuncture treatment in the perioperative period can improve the pain after TKA to a certain extent,although the actual analgesic effect is relatively weak,not as good as the common analgesia.However,this experiment is safe,feasible and operable,and it also has guiding significance for the clinical promotion of floating acupuncture and the research on analgesia after TKA. |
