| Objective: To investigate the effects of lipids(lipids included in the study include:Triglyceride,total cholesterol,high-density lipoprotein cholesterol,low density lipoprotein cholesterol and apolipoprotein A1,apolipoprotein B)were analyzed,in order to determine the blood fat index significance to the diagnosis of differentiated thyroid cancer,further discuss lipid indexes with differentiated thyroid cancer in patients with clinical and pathological features if relevant.Through the analysis of this experiment,it can provide corresponding data support for the future basic research of "the correlation between lipid indexes and differentiated thyroid cancer".Serum lipid index may be a non-invasive adjunct index for diagnosis and clinical evaluation of thyroid adenocarcinoma.Methods: 72 patients with differentiated thyroid cancer who were treated in The Affiliated Hospital of Yan ’an University from December 2014 to January 2020 and met the inclusion criteria were collected as the case group,and 93 patients diagnosed with benign thyroid disease in our hospital during the same period were collected as the control group.Blood lipid indexes(included lipids included: Triglyceride,total cholesterol,high density lipoprotein cholesterol,low density lipoprotein cholesterol,apolipoprotein A1,apolipoprotein B)were compared and analyzed to determine the significance of lipid indexes in the diagnosis of differentiated thyroid cancer.Patients in differentiated thyroid cancer group were divided into subgroups according to their clinical and pathological characteristics,tumor size,histological type,lymph node metastasis status and clinical stage,and their blood lipid and apolipoprotein levels were compared and analyzed.The main blood lipid indexes selected in this experiment include TC,TG,HDL-C,LDL-C,APO-A1 and APO-B.Data related to this experiment were collected and recorded for statistical analysis(SPSS 25.0 statistical software was used).Results: 1.A total of 72 patients with differentiated thyroid cancer and 93 patients with benign thyroid tumor were included in this study.Patients were divided into two groups according to age: < 55 years old group,113 patients,including 45 malignant cases,68 benign cases;There were 52 patients in ≥55 years old group,including 27 malignant patients and 25 benign patients.In terms of gender composition,males accounted for29.2% and females for differentiated thyroid cancer group and males accounted for26.9% and females accounted for 73.1% for benign tumor group.Through comparative analysis,there was no significant difference in age and gender between the case group and the control group,and there was no statistical significance.BMI of differentiated thyroid cancer group(24.10±3.35)was higher than that of benign group(22.95±2.50),and the difference was statistically significant(P<0.05).2.Comparative analysis of lipid index levels between differentiated thyroid cancer case group and benign thyroid control group showed the following results: The levels of TG(1.94±1.56mmol/L)and TC(4.72± 0.88 mmol /L)in differentiated thyroid cancer group were higher than those in benign thyroid control group(1.32± 0.65 mmol /L)and TC(4.02± 0.69 mmol /L),P < 0.05.The difference was statistically significant.The level of Apo A1 in case group(1.00±0,22 mmol/L)was lower than that in benign control group(1.15±0.39 mmol/L).P < 0.05,the difference was statistically significant.There were no significant differences in HDL-C,LDL-C and Apo B between the two groups(P > 0.05).3.Stepwise logistic regression analysis showed that total cholesterol(TC)was positively correlated with differentiated thyroid cancer(P<0.05),and its OR and 95%CI were 2.86,1.72-4.76,respectively.There was a negative correlation between apolipoprotein A1 and differentiated thyroid carcinoma(P<0.05),and the OR value and95%CI were 0.22,0.06-0.76,respectively.It is suggested that the levels of total cholesterol and apolipoprotein A1 are correlated with differentiated thyroid carcinoma.4.Differentiated thyroid cancer group was divided into subgroups according to clinical and pathological features,and the levels of blood lipid and apolipoprotein were analyzed.The results showed that there was no statistical significance in serum lipid and apolipoprotein levels(P > 0.05).Conclusions: 1.Lipid and apolipoprotein levels are associated with differentiated thyroid cancer.Multivariate logistic regression analysis showed that total cholesterol(TC)was positively correlated with differentiated thyroid cancer,while apolipoprotein A1 was negatively correlated with differentiated thyroid cancer.It can be concluded that the levels of total cholesterol and apolipoprotein A1 are correlated with differentiated thyroid carcinoma.3.There was no significant correlation between the levels of blood lipid and apolipoprotein and the clinicopathological characteristics of differentiated thyroid cancer. |
