Exploration Of Clinical Experience In The Treatment Of PCSD-type Prolonged Menstrual Period Based On Chong-Ren Theory

Purpose:This paper aims to explore the etiology,pathogenesis and treatment of uterine incision diverticulum(PCSD)type prolonged menstrual period after cesarean section from the perspective of conditioning Chong-ren,to sort out the theoretical origin of Chong-ren theory,and to provide associate professor Hu Ying’s experience in treating PCSD type prolonged menstrual period.To research and organize,to discuss the syndrome differentiation thinking of this disease and the characteristics of prescription and medication,to summarize the clinical treatment experience,and to broaden the diagnosis and treatment of PCSD-type prolonged menstrual period from the theory of Chong-Ren.Method:In this study,by screening the outpatient and inpatient patients of the instructor,176 cases meeting the diagnostic criteria of PCSD with prolonged menstrual period were selected,and the WPS Excel professional version application software was used to establish a case database,and the collected case data were sorted and classified and imported for analysis.Analysis of age distribution statistics,syndrome keywords,frequency of common syndrome types,frequency of commonly used traditional Chinese medicine application,and drug properties,flavor,and meridians,etc.,through reviewing relevant ancient documents and journal papers,to sort out the theoretical origin of the Chong-Ren theory,and discuss The theoretical basis of using Chong-Ren theory to treat PCSD-type prolonged menstrual period.Analyzed and summarized the instructor’s understanding of the etiology and pathogenesis of PCSD-type prolonged menstrual period,the characteristics of prescription and medication,the thinking of differentiation and treatment,and typical case testing,and summarized clinical experience.Result:1.According to the general age distribution statistics of 176 valid cases,among patients with PCSD-type prolonged menstrual period,with the female Tiangui seven years old as the boundary,up to seven seven(49 years old),down to twenty seven(14 years old),thirty seven to seven years old.The age group of six or seven(22-42 years old)accounts for 85.80%of the incidence population.The disease is mainly directly related to the cesarean section rate,and tends to be younger overall.2.Statistics on the frequency of syndrome keywords of PCSD-type prolonged menstrual period:According to the statistical results,among the 176 cases of PCSD-type prolonged menstrual period,the frequency of syndrome keywords is the highest,and 13 2 cases have virtual keywords.,accounting for 75%of the total,of which 61 cases were mostly pure deficiency syndrome,accounting for 34.66%of the total;followed by keyword deficiency and blood stasis in 28 cases,accounting for 15.91%of the total;keyword deficiency and heat were 26 cases,accounting for 14.7 7%of the total;17 cases were associated with deficiency and stagnation,accounting for 9.66%of the total;63 cases were associated with stasis,accounting for 35.80%of the total,among which stagnation and heat were associated with the keyword stasis,each accounting for 21 There were 14 cases and 14 cases,accounting for 11.93%and 7.96%of the total,respectively;9 cases were related to the keywords depression and heat at the same time,accounting for 5.11%of the total.3.Frequency statistics of syndrome types of PCSD-type prolonged menstrual period:According to the statistical results,the common syndrome types in 17 6 cases of PCSD-type prolonged menstrual period mainly include Qi deficiency and blood stasis syndrome,Yin deficiency and internal heat syndrome,Spleen deficiency syndrome,Qi stagnation syndrome There were 28 cases,26 cases,22 cases,21 cases,17 cases,14 cases,respectively,blood stasis syndrome,liver stagnation and spleen deficiency syndrome,blood stasis dissolving heat syndrome,kidney deficiency syndrome,qi and blood deficiency syndrome,and spleen and kidney deficiency syndrome,respectively.12 cases,10 cases and 10 cases accounted for 15.91%,14.77%,12.50%,11.93%,9.66%,7.96%,6.82%,5.68%and 5.68%,respectively.From this data,it can be seen that the incidence of PCSD-type prolonged menstrual period is closely related to the liver,spleen,and kidney.4.The first prescription of 176 patients with PCSD type prolonged menstrual period was entered into the software for analysis.The results showed that the application frequency of Atractylodes,Codonopsis,Zhigancao,Calcined keel,Calcined oyster,Paeonia lactiflora,Rehmannia glutinosa,Mangosteen meat,and Astragalus were more than 80 times.,sea octopus,gall,ginger,jujube,chuanxiong,red peony,tuckahoe,fern,eucommia,angelica,schisandra,the frequency of application is more than 60 times,Ophiopogon japonicus,donkey-hide gelatin,Bupleurum,Alisma,Motherwort,Mulberry parasitic,cohosh,psoralen,madder application frequency is more than 40 times.5.176 cases of PCSD-type prolonged menstrual period were diagnosed and the first prescription was entered into the software for analysis.It can be seen that the commonly used traditional Chinese medicines in the treatment of PCSD-type prolonged menstrual period are mostly warm and flat,mainly sweet and bitter,and the properties of drugs are ranked in the meridian.The first six are:Liver,Kidney,Spleen,Lung,Heart,and Stomach.Inconclusion:1.The instructor believes that the injury of the golden blade is the main cause of postpartum blood loss and qi injury,and the pathological factors are mainly due to stasis and stagnant heat.Chong and Ren disorders,qi deficiency is an important pathogenesis,qi and blood disharmony,viscera deficiency are the basic pathological changes.2.The instructor is good at analyzing the disease based on the theory of Chong-Ren in traditional Chinese medicine,emphasizing the pathogenesis of Chong-Ren disorder,advocating the benefit of the innate and the acquired,at the same time paying attention to the balance of qi,blood,yin and yang,and putting forward the corresponding syndrome differentiation characteristics for different syndrome types.Special attention is paid to the use of classic prescriptions in the formulation of prescriptions,such as Xiaoyao Pills,Juyuanjian,Gu Chong Decoction,Bushen Gu Chong Decoction,Liangdi Decoction,etc.Fill essence,qi and blood mainly.3.Summarize the essentials of the instructor’s clinical diagnosis and treatment,namely:expounding the advantages of the Chong-Ren theory in the diagnosis and treatment of gynecological diseases,emphasizing the reproductive axis of "Kidney-Tiangui-Chong-Ren-Uterus";women take Chong-Ren as the core,and the principle of adjusting Chong-Ren runs throughout.;Emphasizes the basis of traditional Chinese medicine and the concept of
