| Objective:We retrospectively analyzed the epidemiological characteristics of elderly burn patients in Jiangxi Burn Center from January 1,2014 to December 31,2021.The study aimed to provide ideas for clinical treatment and prevention of elderly burn.Methods:Hospital data of elderly burn patients who met the inclusion criteria and admitted to Jiangxi Burn Center from January 1,2014 to December 31,2021 were collected and analyzed.The demographic characteristics,injury situation,injury characteristics,treatment and outcome of elderly burn patients were statistically analyzed.Data was processed with chi-square test,Fisher exact probability test,Kruskal-Wallis H test,Mann Whitney U test,and Spearman’s Rank Correlation.Results:1.There were 836 elderly burn patients in Jiangxi Burn Center from January 1,2014 to December 31,2021,which accounting for nearly 7.8%of total burn patients over the same period.A total of 796 elderly burn patient hospital data were collected in this study(40 patients with less than 24 hours of hospitalization were excluded).The age distribution ranged from 65 to 97 years old,with patients aged 65-74 years were the most(57.6%).Males more affect than females,with a ratio of 1.15:1,There are differences in the gender distribution of elderly patients with different injury causes and different burn degrees,with statistical significance(X~2=31.20,X~2=9.012,P<0.05).With the increase of age,the proportion of rural patients with senile burns decreased,and the difference was statistically significant(X~2=9.753,P<0.05).2.The main cause of burn injury in elderly patients was flame(57.0%).There were significant differences in the distribution of injury causes in different aged elderly patients(c~2=16.285,P<0.05).The incidence of elderly burns was highest in summer,accounting for 35.6%,and the proportion of different causes of burn injury was different in four seasons,with statistical significance(c~2=86.377,P<0.05).There were 37.3%patients who were admitted to hospital over 48 hours after injury,and 62.7%patients who were admitted to hospital within 48 hours after injury.3.The elderly patients were mainly moderately burned(54.4%).The total burn area was 9%(5%,18%)TBSA.The incidence of third-degree burn was 64.3%,and the area of third-degree burns was 2%(0%,6%)TBSA.The total burn area and third-degree burns area among the three ages groups were similar without statistically significance(H=4.246,H=3.906,P>0.05).4.The elderly burn patients with pre-injury systemic disease accounted for65.7%,the incidence of cardiovascular diseases was the highest(43.0%),the followed were central nervous system diseases and endocrine system diseases.The highest incidence of pre-traumatic diseases was hypertension(37.1%).5.Elderly burn patients are mainly treated by conservative dressing change,with the increase of age,the proportion of burn patients who choose conservative dressing change increased,and the difference was statistically significant(c~2=22.939,P<0.05).The length of stay(LOS)in hospital of elderly burn patients was ranged from 1 to145 days,and the hospitalization cost ranged from 1406.0 to 750082.2 yuan.The hospitalization time and hospitalization cost of elderly burn patients with different treatment methods were significantly different,with statistical significance(Z=-15.548,Z=-17.074,P<0.05).6.The cure rate of elderly burn patients was 41.5%,the improvement rate was35.2%,and 4.0%of patients died in hospital.With the increase of age and the aggravation of burn severity,the cure rate of elderly burn patients decreased and the mortality rate increased,and the difference was statistically significant(H=20.883,r_s=0.372,P<0.05).There were 17.0%of elderly patients with inhalation injury,and the mortality rate was 14.1%,which was significantly higher than that of patients without inhalation injury(Z=-18.378,P<0.05).The cure rate of the elderly burn patients who chose surgical treatment was higher than that of the conservative dressing change group,and the difference was statistically significant(Z=-2.109,P<0.05).The mortality of elderly burn patients with different number of pre-injury systemic disease was different,and the difference was statistically significant(r_s=0.029,P<0.05).ConclusionThe proportion of elderly burn patients in the total burn patients in the same period is increasing,with males more than females,rural patients more than urban patients,and patients aged 65-74 years are the main group.Elderly burns are mainly moderate burns and more than half of elderly burn patients complicated with pre-injury systemic disease.Elderly burns occur most frequently in summer,and the most common cause of injury is flame.The effect of surgical treatment is better than that of conservative dressing change.Age,burn degree,inhalation injury and pre-injury systemic disease are the risk factors for death of elderly burn patients. |
