| Objective:The correlation between serum uric acid(UA),cystatin C(CysC),β2microglobulin(β2M)and elderly patients with essential hypertension left ventricular hypertrophy(LVH)was analyzed retrospectively,providing partial theoretical basis and monitoring indicators for early detection,diagnosis and intervention of elderly patients with essenti al hypertension LVH.Methods:Selected: A total of 217 elderly hypertensive patients who were hospitalized in the first,second and third groups,the cardiac intensive care unit(CCU)and the elderly cadre group of the Second Hospital of Shanxi Medical University from January 2019 to January 2021.Collect and summarize general medical history data: gender,age,height,weight,family history and smoking history etc,according to height and body mass index(BMI)and body surface area(BSA).Blood indexes UA,CysC,β2M,creatinine(Cr),urea(Ur),high density lipoprotein(HDL),triglyceride(TG),low density lipoprotein(LDL),total cholesterol(TC)etc,were detected in the laboratory department of the second hospital.All selected patients completed ordinary two-dimensional echocardiography to measure ventricular septal thickness(IVST),left ventricular end-diastolic diameter(LVEDD),left ventricular posterior wall thickness(LVPWT),and then calculated left ventricular mass index(LVMI).Patients were divided into LVH group(n=97)and NLVH group(n=120)according to LVMI criteria.Statistical methods such as t test,Mantler-Wheeney U test and chi-square test were used to analyze the differences of general medical history data and blood biochemical data such as UA,CysC,β2M between LVH group and NLVH group with essential hypertension.Binary logistic regression and pearson correlation test were used to analyze the relationship between related indexes and LVH in senile essential hypertension.Diagnostic and predictive value of ROC curve evaluation of relevant indicators.Results:The two groups of elderly hypertensive patients included in the study had no significant differences in basic clinical data gender,body mass index(BMI),and smoking history,and the difference was not statistically significant(P>0.05).There was a Significant differences between age,systolic blood pressure(SBP)(P<0.05);among the general biochemical indicators,creatinine(Cr),urea(Ur),low-density lipoprotein(LDL),high-density lipoprotein(HDL),triglyceride(TG),total cholesterol(TC),there was no significant difference between the two groups(P>0.05).Uric acid(UA),cystatin C(CysC),β2 microglobulin(β2M)in the LVH group were significantly higher than those in the NLVH group(P<0.05).Pearson correlation analysis showed that UA,CysC,β2M were positively correlated with left ventricular mass index(LVMI),the correlation coefficients r are: UA=0.359,CysC=0.379,β2M=0.421,P<0.05.The results of binary logistic regression analysis showed that UA(OR=1.019,P<0.05),CysC(OR=8.957,P=0.032,β2M(OR=3.364,P=0.001)were associated with left ventricular hypertrophy(LVH)in elderly hypertensive patients.The area under the curve of ROC curve for predicting LVH in elderly hypertensive patients: UA=0.778,CysC=0.653,β2M=0.680,ROC curve predicted by the three indicators combined was 0.824.Conclusion:1.Serum uric acid(UA),cystatin C(CysC),and β2 microglobulin(β2M)are positively correlated with the increase of left ventricular mass index(LVMI)in elderly patients with essential hypertension;all three are independent risk factors for left ventricular hypertrophy(LVH)in elderly patients with essential hypertension.2.UA,CysC and β2M have certain predictive value for elderly patients with essential hypertension complicated with left ventricular hypertrophy.Compared with CysC and β2M,UA has higher predictive value.Compared with any single index,the combination of the three indicators of UA CysC and β2M has improved the predictive value and specificity,and the simultaneous determination of the three indicators can better predict and evaluate the occurrence and development of LVH. |
