| Objective:1.Exploring Professor Yuan Huishu ’s medication rules in the treatment of chronic heart failure by TCM Inheritance Auxiliary Platform,select core drugs and provide reference for clinical treatment of chronic heart failure.2.Exploring the mechanism of treating chronic heart failure with the screened core drugs by using network pharmacology method,to provide data support and literature basis for the experimental direction of traditional Chinese medicine in the treatment of chronic heart failure.Methods:1.Collect cases meeting the criteria of inclusion and discharge treated by Professor Yuan from outpatient clinics of the Second Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine and Hainan Branch from January 1,2016 to October 1,2021,standardize the data and input the sorted case information into the TCM Inheritance Auxiliary Platform(V2.5)software to establish a database on Professor Yuan Huishu ’ s treatment of chronic heart failure.The clinical experience of Professor Yuan Huishu in the treatment of chronic heart failure was summarized by using the function of frequency statistics,prescription rules and new prescription analysis.2.The active components and targets of core drugs were screened,and the intersection targets of core drugs and chronic heart failure were obtained.The core drugs-active components-target network was constructed used by Cytoscape 3.9.0.Upload the intersection targets to the String database to construct the protein interaction network.Import the intersection targets into the Metascape database for GO and KEGG enrichment analysis.To predicte and discuss the potential mechanism of core drugs in the treatment of chronic heart failure.Results:A total of 200 cases of chronic heart failure were included,including 123 females and 77 males.The number of patients aged 70-80 is the highest(46%).The three syndromes with the highest occurrence frequency are qi deficiency and blood stasis syndrome combined with phlegm retention syndrome,qi and yin deficiency and blood stasis syndrome combined with phlegm retention syndrome,and qi deficiency and blood stasis syndrome.In this study,a total of158 common clinical symptoms are involved.The top 15 clinical symptoms are edema of lower limbs,purple and dark tongue,chest tightness,shortness of breath,shortness of breath,shortness of breath,blood stasis in tongue,pulse string,palpitation,ecchymosis in tongue,fatigue,constipation,frequent urination,inability to lie down,night urine frequency and pulse delay.A total of 152 kinds of Chinese medicine,the most frequent occurrence of the four nature of drugs is warm nature,followed by cold,flat,cool and hot;the highest frequency of five flavors is sweet,followed by bitter,pungent,sour,salty;the highest fre-quency of meridians is the lung meridian,followed by liver,spleen,heart,stomach and kidney.The highest frequency of 10 kinds of tradi-tional Chinese medicine were: poria,atractylodes,tangerine peel,semendescurainiae,radix stephaniae tetrandrae,bupleurum,membranaceus,ginseng,salvia,notoginseng.The commonly used drug combinations wereatractylodes-poria,tangerine peel-atractylodes-poria,atractylodes-radix stephaniae tetrandra-poria-semen descurainiae,tangerine peel-atractylodes-radixstephaniae tetrandrae-poria-semen descurainiae.The high confidence associationrules are "atractylodes→poria ","tangerine peel,atractylodes→poria","bupleurum,poria,semen descurainiae→atractylodes","tangerine peel,radix stephaniae tetrandrae,poria,semen descurainiae→atractylodes";a total of 16 potential core drug combinations and 8 potential new prescriptions were extracted by entropy hierarchical clusteringalgorithm.Traditional Chinese medicine(TCM)with the frequency of ≧60% was used as the core drug in the data mining results,and the frequency from high to low was poria,atractylodes,tangerine peel,semen descurainiae,radix stephaniae tetrandrae,bupleurum,membranaceus,ginseng.After screening,59 active components and 242 drug targets were obtained,and 121 drug targets were obtained after crossing with1382 disease targets.The key active components included quercetin,kaempferol,chuanchenpietin,naringin,β-sitosterol,isorhamnetin and formononetin.There were 121 nodes and 2436 edges in the protein inte-ractionnetwork.11 key core targets were screened,including IL-6,AKT1,TNF,IL-1β,VEGFA,TP53,CASP3,MMP9,JUN,CXCL8 and PTGS2.GO functional enrichment analysis obtained the main biological processes related to chronic heart failure included response to lipopolys-accharide,response to trauma,blood circulation,apoptosis signaling pathway,and positive regulation of protein phosphorylation.A total of 331 pathways were obtained by KEGG pathway enrichment analysis,among which the key pathways were AGE-RAGE signaling pathway,IL-17 signaling pathway,MAPK signaling pathway,estrogen signaling pa-thway,and VEGF signaling pathway.Conclusion:1.Data mining of medication rules shows that chronic heart failure is located in the heart,involving lung,liver,spleen,kidney and other organs,and its basic pathogenesis is "Qi deficiency and blood stasis and water stop".Professor Yuan Huishu’s core prescription drugs forchronic heart failure include poria,atractylodes,tangerine peel,semendescurainiae,radix stephaniae tetrandrae,bupleurum,membranaceus,ginseng,salvia,notoginseng,os draconis,concha ostreae.Professor Yuan Hui shu will always carry out the law of "replenishing qi,promoting blood circulation and promoting diuresis".2.Network pharmacology studies have shown that the mechanism of action of core drugs in the treatment of chronic heart failure may be quercetin,kaempferol,tangerine,naringin,β-sitosterol,isorhamnetin,formononetin and other active components.By influencing the targets such as IL-6,AKT1,TNF,IL-1β,VEGFA,TP53,CASP3,MMP9,JUN,CXCL8,PTGS2.To regulate AGE-RAGE signaling pathway,IL-17 signaling pathway,MAPK signaling pathway,estrogen signaling pathway,VEGF signaling pathway,involved in anti-inflammatory,anti-oxidative stress,anti-apoptosis,reduce myocardial fibrosis and other biological processes. |
