| Objective:1.Through the data mining method,we explored the drug use pattern of heart yang deficiency syndrome in chronic heart failure,analyzed the frequency of drugs,the nature and taste of drugs,and the combination of drugs,summarized the core prescriptions for the treatment of heart yang deficiency syndrome in chronic heart failure.Finally,The research derived the overall treatment principles and treatment methods.The research provided feasible treatment plans for the treatment of Chinese medicine,and at the same time also provided ideas for further clinical observation trials,and provided strong supporting evidence for the next basic experimental research.2.Through the research method of network pharmacology,we predicted the grouping rules,medication principles,potential targets and pathway mechanisms of the core prescriptions obtained from data mining for the treatment of heart failure in heart yang deficiency syndrome.The research provided a solid theoretical basis for future research on the prevention and treatment of heart yang deficiency syndrome of heart failure and a reference value for future clinical use.Methods:1.Data miningAfter screening the complete medical records from September 2020 to January 2023 from the outpatient cardiology department of the Third Affiliated Hospital of Beijing University of Chinese Medicine,all the data in the medical records were imported into an Excel sheet.We standardized this data and performed two successive proofreading.Then the screened medical records were subjected to data striping,database building,secondary standardization of data,data analysis,and information mining by using the TCM heritage assistance platform software.Through the process of general condition analysis,drug frequency statistics,and association rule analysis of the medical records,we explored the common drug pairings and core prescriptions for the treatment of chronic heart failure with heart yang deficiency syndrome.2.Network pharmacologyThe core prescriptions obtained from data mining were retrieved through the research strategies and methods of network pharmacology for the main components and related target information.We screened and intersected the target information with the disease targets of heart failure.Then the common targets obtained were used to establish PPI networks for enrichment analysis of GO and KEGG pathways,which were finally visualized and analyzed through visualization software to provide certain reference directions for later basic experimental research exploring the mechanisms of TCM against heart failure.Result:1.The result of Data miningThis chapter utilizes the TCM inheritance aid platform to explore and summarize the medication pattern of chronic heart failure in the heart-yang deficiency syndrome.During the study,221 medical records were included,and the prescriptions covered 166 flavors of Chinese medicine.We concluded that the medication pattern for the treatment of chronic heart failure in the heart-yang deficiency syndrome is based on warming and regulating the heart,supplemented by cold and flat properties,focusing on the taste of pungent and sweet to transform yang and bitter to enter the heart meridian,and also focusing on the overall regulation of the viscera.It mainly enters the heart and spleen meridians,and smoothly regulates the lung,stomach and liver meridians.After data extraction and analysis,the frequency of 22 herbs was greater than or equal to 20 times,with atractylodes macrocephala 216 times,ligusticum wallichii 213 times,salvia miltiorrhiza 208 times,cassia twig 203 times and cinnamomum cassia 196 times being the most high frequency;secondly,based on the association rule algorithm,we analyzed the combination pattern of 2-4 different drugs,and the high frequency pairs were concentrated in poria cocos,cinnamomum cassia,atractylodes macrocephala,codonopsis pilosul,ligusticum wallichii,cassia twig and pericarpium arecae.The core drug network result graph shows that according to the four different levels of support adjusted,the prescription pattern under this support level was obtained,with the increasing support level,the peripheral discrete drugs gradually disappeared,the middle core drug group also became clearer,and the commonality between drug combinations also became more prominent.Finally when the support level was adjusted to 75%,the core prescription of treatment was concluded to be composed of poria cocos,cinnamomum cassia,atractylodes macrocephala,codonopsis pilosul,ligusticum wallichii,salvia miltiorrhiza,cassia twig and pericarpium arecae 8 drugs.2.The result of Network PharmacologyBy searching the database for the ingredients of 8 drugs in the core prescription,we obtained 112 active ingredients and 176 drug-related targets.By searching the disease-related database,a total of 39184 heart failure disease targets were obtained.Then we intersected the obtained drug and disease targets and established the drug-disease target interaction network diagram.A total of 138 drug-disease intersection targets were obtained,and the common targets were GO and KEGG enrichment analyses were performed.All the results were integrated to further screen.We find12 active ingredients,6 pathways and 7 primary targets for therapeutic effects,and the final results were visualized.Conclusion:The data mining results summarized that the core prescription is composed of 8 drugs:poria cocos,cinnamomum cassia,atractylodes macrocephala,codonopsis pilosul,ligusticum wallichii,salvia miltiorrhiza,cassia twig and pericarpium arecae.The network pharmacology results inferred that the composition of the core prescription is composed of codonopsis pilosul as the ruler,salvia miltiorrhiza,cassia twig and cinnamomum cassia as the minister,together with atractylodes macrocephala,ligusticum wallichii,poria cocos,and pericarpium arecae to exert therapeutic effects.The preliminary indication of the active ingredients are apigenin,luteolin and oleic acid,while the secondary active ingredients 7-Methoxy2-methyl isoflavone,Stigmasterol and tectorigenin play a synergistic role.The signaling pathways that exert therapeutic effects are mainly the interleukin pathway,PI3K-Akt signaling pathway,HIF-1 signaling pathway p53 signaling pathway play a synergistic role.The main targets are RXRA,CHRM1,IL6,and the secondary targets are BCL2,CASP9,CCND1,CDKN1A. |
