| Objective: The main purpose of this study is to observe the clinical effect of acupotomy on primary facial spasm,in order to provide a new idea and method for the clinical treatment of primary facial spasm. Methods: 60 patients with primary hemifacial spasm were randomly divided into two groups. In the control group, ordinary acupuncture therapy was used, and the affected side:Yifeng,Yangbai, cuanzhu,Yangyang, quanliao,Fengchi points and bilateral Hegu and Taichong points were taken. The treatment group was treated with acupotomy therapy, fixed point: 1.Trigger point(tenderness point or the place where the spasm can be relieved by pressing);2.Stem mammary foramen(between styloid process and mastoid process);3.Highest point of zygomatic process;4.Midpoint of zygomatic arch;5.Bone surface of posterior zygomatic arch;6.Zygomatic arch origin site;7.10 mm behind(lateral)pulse of facial artery;8.The anterior(medial)pulse of facial artery is 10mm;9.Maxillary bone surface of upper lip;10.Eyebrow midpoint slightly on the bone surface;11.Lateral eye fissure of eyebrow arch;12.The inner end of the eyebrow arch. The control group was treated with acupuncture once a day for six consecutive days and a day’s rest for two weeks as a course of treatment. The treatment group received acupotomy every other week, and every two times was a course of treatment.Patients in both groups were treated for two consecutive courses.During the treatment, the intensity and frequency of patients’ facial muscle spasm were recorded, and the clinical manifestations of patients in the two groups were compared before and after the treatment with the classification standard of Intensity and frequency of Cohen facial muscle spasm as an indicator,so as to evaluate the clinical effect.Results:1.Grade comparison of spasm intensity and frequency:(1)In the treatment group, the symptoms of facial spasm were improved immediately after acupotomy, indicating that acupotomy had immediate effect.(2)Both treatments could effectively improve the intensity and frequency of facial spasm(P< 0.05),but there was no significant difference between the two groups(P > 0.05).2.Comparison of clinical efficacy:After the end of all treatment courses,in the treatment group, there were 5 cases of cured,10 cases of marked effect,12 cases of effective and 2 cases of ineffective, with a total effective rate of 93.1%,while in the control group, there were 4 cases of cured,8 cases of marked effect,14 cases of effective and 4 cases of ineffective, with a total effective rate of 86.7%.There was no statistical difference in symptom efficacy between the two groups(P>0.05).Conclusion:1.Acupotomy has immediate effect on hemifacial spasm.2.Acupotomy is an effective method to treat hemifacial spasm.3.Acupotomy is simple and reliable, which can provide a new idea and method for clinical treatment of hemifacial spasm. |
