Clinical Efficacy Of Sitz Bath With Xiaoyong Formula In The Treatment Of Postoperative Wound Healing Of Perianal Abscess With The Severe Heat Toxin Syndrome

ObjectiveTo analyze the distribution of pathogenic bacteria of perianal abscess and to explore the clinical efficacy of Xiaoyong formula sitz bath on postoperative wounds of perianal abscess.MethodologyFrom January 2021 to January 2022,60 patients with perianal abscesses diagnosed as the severe heat toxin syndrome were selected from the Department of Anorectal Medicine of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine.The random number table method was used to randomly divide into 2 groups: 30 cases in the treatment group and 30 cases in the control group.Patients in both groups were routinely subjected to preoperative examination,preoperative preparation,pus microbial culture,and perianal abscess incision and drainage;Postoperative treatment includes traditional Chinese medicine sitz bath,dressing change,routine laxative,anti-inflammatory,analgesic,hemostasis and other symptomatic treatments.The control group received a sitz bath with compound phellodendron liquid post-operative,while the treatment group received a sitz bath with Xiaoyong formula post-operative.The initial wound pain and area score of the two groups at rest were recorded before sitz bath on the first day after operation.The trauma pain score,trauma area,trauma exudate and granulation growth were recorded on postoperative day 7,day 14 and day 21 in both groups.The clinical efficacy and recurrence of patients in both groups were objectively assessed,and safety evaluation was performed.Results1.General data comparison: the analysis of the two groups of patients in terms of age,gender,course of disease,initial trauma pain score,and initial trauma area showed no statistically significant differences(P > 0.05),indicating that the data were comparable.2.The analysis of the microflora distribution of the two groups showed that the gram-negative bacterium Escherichia coli is the main pathogenic bacteria causing perianal abscesses,and the antibiotic application can be reasonably adjusted for this pathogenic bacteria clinically.3.Comparison of clinical efficacy:(1)Wound pain: on postoperative days 7 and 14,there was a statistical difference in the trauma pain scores between the two groups(P < 0.05),with the treatment group outperforming the control group;(2)Wound granulation : on postoperative days 7 and 14,there was a statistical difference in the wound granulation growth scores between the two groups(P < 0.05),with the treatment group outperforming the control group;(3)Wound exudate: on postoperative days 7 and 14,there was a statistical difference in the comparison of wound exudate between the two groups(P < 0.05),with the control group outperforming the control group;(4)Wound area: on postoperative days 14 and21,there was a statistical difference in the wound area and area reduction rate between the two groups(P < 0.05),and the treatment group was better than the control group.(5)Wound healing time: there was a statistically significant difference(P < 0.05)between the two groups in terms of the time of complete wound healing,and the treatment group was better than the control group;(6)Efficiency rate: after 21 days of treatment,the efficiency rate of both groups was 100%;the significant rate was 2% in the treatment group and 0 in the control group,and there was no statistically significant difference(P >0.05)between the clinical efficacy of the two groups;(7)Ecurrence rate: at follow-up 3 months after the end of treatment,patients were recurrence-free,and there was no statistical difference in the recurrence rate between the two groups;(8)Comparison of safety: no significant toxic side effects and adverse reactions such as skin allergy,eczema and erythema were observed during the treatment in both groups.Conclusion1.The main pathogenic bacteria of perianal abscess is Escherichia coli.2.Xiaoyong formula Sitz bath can reduce postoperative wound pain,promote granulation tissue growth and accelerate wound healing in patients with perianal abscess,with remarkable efficacy and good safety,which is worthy of clinical promotion.
