| Objective:By detecting the levels of brain-derived neurotrophic factor(BDNF)and uric acid(UA)in serum of patients with Parkinson’s disease(PD),the relationship between the levels of BDNF and UA in serum and the severity of PD and Parkinson’s disease with depression(PD-D)was discussed.and further explore whether serum BDNF and UA have certain predictive and diagnostic effects on the occurrence of PD,providing a certain basis for the study of peripheral blood biomarkers of PD.Methods: 62 PD patients were selected as PD group,and 30 healthy people were selected as control group.The subjects in PD group were evaluated by H-Y grading(H-Y grading)and Hamilton Depression Scale(HAMD).The PD group was divided into two subgroups according to H-Y grading: early PD group and mid-to-late PD group.According to HAMD score,PD components were divided into two subgroups:PD-D group and non-PD-D group.General data and blood samples were collected from all subjects.Enzyme-linked immunosorbent assay was used The serum levels of UA and BDNF in all subjects were detected by assay(ELSIA)and uricase method,respectively.The differences of serum levels of BDNF and UA between PD group and control group were analyzed,and the serum levels of BDNF and UA among PD group subgroups were compared and analyzed.The correlation of serum BDNF and UA levels with H-Y grading,HAMD score,age and other clinical characteristics in PD group was observed.Finally,Receiver operating characteristic curve(ROC)was used to analyze the diagnostic efficacy of serum BDNF and UA levels on PD,to determine whether serum BDNF and UA have certain predictive and diagnostic effects on the occurrence of PD.Results: 1.Comparison of general clinical data: The PD group and the control group were compared in terms of age,gender,body mass index(BMI),creatinine(Cr),the endogenous creatinine clearance(Ccr),etc.There was no significant difference(P > 0.05).In the two subgroups divided by H-Y grading,the disease course of early PD group was significantly shorter than that of middle and late PD group(t=-3.10,P < 0.05).There were no significant differences in age,gender,BMI,Cr and Ccr between the two groups(P > 0.05).In the two subgroups divided by HAMD score,there was no significant difference in age,gender,BMI,Cr,Ccr,disease duration and other aspects between the two groups(P > 0.05).2.When analyzing the serum BDNF level of all subjects,it was found that the serum BDNF level of PD group(1930.39±565.49ng/L)was significantly lower than that of control group(2700.32±511.93ng/L)(t=-6.31,P < 0.001).In the comparison between the early PD group and the mid-to-late PD group,The serum BDNF level in the mid-to-late PD group(1625.73±554.78ng/L)was significantly lower than that in the early PD group(2165.42±455.84ng/L)(t=4.21,P < 0.001).The serum BDNF level in PD-D group(1653.32±445.03ng/L)was lower than that in non-PD-D group(2434.15±386.00ng/L)(t=-6.92,P < 0.001).The correlation analysis of serum BDNF level with H-Y grading,HAMD score and age in PD group showed that serum BDNF level was negatively correlated with H-Y stage,HAMD score and age in PD group(r < 0,P < 0.05).In the analysis of the diagnostic efficiency of serum BDNF for PD,the ROC area under the curve(AUC)of serum BDNF in the diagnosis of PD was 0.851,P value < 0.001,and its critical value was 2315.83ng/L,The sensitivity and specificity were 80.60%and 90.00%,respectively.2.When the serum UA level of all subjects was analyzed,it was found that the serum UA level of PD group(285.03±41.76μmol/L)was significantly lower than that of control group(374.64±79.56μmol/L)(t=-7.10,P < 0.001).In the comparison between the early PD group and the mid-to-late PD group,The serum UA level in early PD group(251.17±28.24μmol/L)was significantly lower than that in mid-to-late PD group(311.16±30.09μmol/L)(t=7.99,P < 0.001).There was no significant difference in serum UA level between PD-D group and non-PD-D group(P > 0.05).In the correlation analysis,it was found that serum UA level in PD group was negatively correlated with H-Y grading(r < 0,P < 0.001),but not with HAMD score and age(P > 0.05).When analyzing the efficacy of serum UA level in the diagnosis of PD,the AUC of serum UA in the diagnosis of PD was 0.828,P value < 0.001.When the critical value was 335.91μmol/L,the sensitivity and specificity were 87.10% and 66.70%,respectively.Conclusions: 1.Serum BDNF and UA levels are significantly correlated with PD.Serum BDNF and UA levels are decreased in PD patients,which may be related to the pathogenesis of PD.2.Serum BDNF and UA levels in patients with advanced PD are lower than those in patients with early PD.Serum BDNF,UA level to a certain extent,can reflect the severity of the PD 3.Serum BDNF level is significantly correlated with PD-D.Serum BDNF level in PD-D patients decreases and decreases with the increase of depression degree.There is no significant correlation between serum UA level and PD-D in PD patients.4.Serum BDNF and UA may have certain predictive and diagnostic effects on the occurrence of PD. |
