Study Of The Family Management Intervention On 1 To 6 Year-Old Children With Nephrotic Syndrome

ObjectiveTo explore the influence of family management intervention on the management ability,family function,quality of life,dietary knowledge of the diseases,medical compliance,infection and recurrence rate in children with nephrotic syndrome.MethodsThis study is a quasi-experimental study.A total of 106 families of children with nephrotic syndrome aged 1-6 years old who were admitted to a tertiary hospital in Qingdao from January 1st,2019 to December 31st,2020 were selected as the research subjects.The families of children with nephrotic syndrome who were hospitalized in 2019were the control group,and the families of children with nephrotic syndrome who were hospitalized in 2020 were the intervention group,of which 53 families were the control group and 53 families were the intervention group.Questionnaires and scale surveys were conducted within 24 hours after admission in both groups,including general conditions of children and their families,dietary knowledge of the disease,family management scale,family function rating scale,and the World Health Organization Quality of Life Scale-Brief Form.The control group received routine treatment,nursing,health education and discharge guidance.The researcher group set up a family management intervention.In addition to routine treatment,nursing,health education and discharge guidance,the intervention group also used a family management intervention for the intervention group,including family management ability,family function and dietary knowledge of nephrotic syndrome.The intervention time was from 24 hours after admission to 3 months after discharge.The intervention methods included presentation,distribution of family management manual,health education,video teaching,peer communication and outpatient supervision.Both groups were investigated using the same questionnaires and scales when they were discharged after 3 months.The study also compared the length of hospital stay,infection rates and relapse rates between the two groups of children.Data were entered by Excel 2010 and analyzed by SPSS 21.0.Statistical analysis used descriptive analysis,two independent samples t-test,paired t-test and Chi-square test.Results(1)Comparison between the two groups before the intervention:comparison of primary caregivers,observation indicators between the two groups:no statistical difference in family management ability,family function,mastery of dietary knowledge of disease and the quality of life(P>0.05).(2)Comparison of control group before and after intervention:comparison of primary caregivers,observation indicators:family management ability,family function,mastery of dietary knowledge of disease and the quality of life were not obvious and there was no statistical significance(P>0.05).(3)Comparison before and after intervention within the intervention group:comparison of primary caregivers,observation indicators:family management ability,family function,mastery of dietary knowledge of disease and the quality of life were all statistically significant(P<0.05).(4)Comparison of two groups after intervention:for primary caregivers,there were statistically significant differences in family management ability,family function,mastery of dietary knowledge of disease and the quality of life between the two groups after intervention(P<0.05).For children with nephrotic syndrome,hospitalization time,infection rate and recurrence rate in last 3 months were statistically significant(P<0.05).There was no statistical difference in drug compliance between the two groups(P>0.05).Conclusions(1)The application of family management intervention in the nursing of children with nephrotic syndrome can improve the primary caregivers,knowledge level of nephrotic syndrome and family management ability,and effectively promote the improvement of family function and quality of life.(2)The application of family management intervention in the nursing of children with nephrotic syndrome can effectively shorten the length of hospital stay and effectively reduce the infection and recurrence rate of children with nephrotic syndrome.But the application of family management mode did not show a significant effect on improving medication compliance of children with nephrotic syndrome.
