| Objective:To study the rules of acupoints selection in the treatment of post-stroke depression by acupuncture,and combined with the structural anatomy of acupoints,to study the nerve conduction mechanism of acupuncture in the treatment of post-stroke depression,in order to provide some evidence and theoretical support for the clinical treatment of post-stroke depression.Methods:Study one.The literature of clinical randomized controlled trials with clear efficacy of acupuncture in the treatment of post-stroke depression was searched by computer.The databases of Wanfang,Sino Med,CNKI,VIP,Pub Med,Web of science and Embase were searched.The retrieval time limit was from the establishment of the database to November 8,2021.According to the inclusive and exclusion criteria,the literatures were screened and the quality of the included literatures were evaluated.The data included in the literature was extracted and imported into Microsoft Excel 2021 to establish the "acupoint database for acupuncture treatment of post-stroke depression".Microsoft Excel 2021 was used to do descriptive statistical analysis;The IBM SPSS modeler 18 was used to perform association analysis on all acupoints by Apriori algorithm;the square Euclidean distance under SPSS statistics 21 binary variable was used as the measurement method,and the ward connection method was used to perform cluster analysis on high-frequency acupoints;Extract the acupoints which were in strong correlation,and were used more than 90 times,and were classified into the same group by cluster analysis,as the core prescription for acupuncture treatment of poststroke depression.Study two.Structure the "anatomical database of acupoints structure".The relevant anatomical resources in the National Library of China,the Library of the Chinese Academy of Sciences,the Medical Library of Peking University and the National Library of the United States were searched by computer.The retrieval time limit was from the establishment of the museum to November 8,2021.Using Microsoft Excel 2021 software,according to the distribution characteristics,basic information and hierarchical structure of acupoints,the database directory was constructed,the database sub table was constructed for the positions of acupoints,and the main body of the database was constructed for the structural anatomy of acupoints.Based on the frequency statistics and association rules analysis results of acupoints used in Study one,the anatomical structure of acupoints used more than 30 times and supported more than 15% in the database was extracted,and the cutaneous nerve,sympathetic nerve,muscular nerve and their branches were analyzed.The characteristics of neuro dissection structure of acupoints were summarized,and the possible neural conduction mechanism of acupuncture in the treatment of post-stroke depression was studied.Results:Study one1.A total of 214 literatures are included.The quality evaluation of the Jadad improved literature shows that 199 literatures are in high-quality and 15 literatures are in low-quality.2.A total of 173 acupoints are involved in the acupuncture prescription,with a total frequency of 2199.The high-frequency acupoints are Baihui,Taichong,Neiguan,Shenmen,Sanyinjiao,Yintang,Sishencong,Zusanli,shenting and Hegu;The number of acupoints used in foot Shaoyang gallbladder meridian ranks first;The meridians with the highest frequency of acupoint use are Du Meridian and foot Jueyin liver meridian;Acupoints are mostly distributed at the ends of limbs and head and face;Most of the specific acupoints are original acupoints.3.Correlation analysis shows that Neiguan and Baihui,Shenmen and Baihui,Sanyinjiao and Baihui,Yintang and Baihui are supported by more than 45.7%,which were commonly used acupoints.4.Three groups are obtained by systematic cluster analysis: Baihui,Taichong,Shenmen and Neiguan;Sanyinjiao,Yintang,Hegu,Sishencong,Zusanli and shenting;Xinshu,Pishu,Shenshu,Ganshu,Fenglong,Qimen,Taixi,Tanzhong,Shuigou.The core prescriptions for PSD are Baihui,Taichong,Shenmen and Neiguan.Study two1.Based on 18 acupoint structural anatomy resources,the acupoint structural anatomy database was established.The database involves 3 directories,1 main table,3 sub tables,13 bytes,362 meridians and 47 extra meridians.2.Distribution characteristics of cutaneous nerve at common acupoints of post-stroke depression: the cutaneous nerves at the head and face acupoints originate from the trigeminal nerve and cervical plexus,the cutaneous nerves at the end of the upper limbs acupoints originate from the brachial plexus,the cutaneous nerves at the end of the lower limbs acupoints originate from the lumbar plexus and sacral plexus,and the cutaneous nerves at the trunk acupoints originate from the corresponding spinal nerves,which are segmental.3.The distribution characteristics of the muscle nerves of common acupoints for poststroke depression: the muscle nerves of the head and face acupoints originate from the facial nerve,the muscle nerves of the upper extremity acupoints originate from the brachial plexus,the muscle nerves of the lower extremity acupoints originate from the sacral plexus,and the muscle nerves under the trunk acupoints originate from the spinal nerves of different segments.4.The distribution characteristics of sympathetic nerves under common acupoints for poststroke depression: the postganglionic fibers of sympathetic nerves at the head and face acupoints originate from the superior cervical sympathetic ganglia,the sympathetic nerves at the end of the upper limb acupoints originate from the stellate ganglia and the 1st,2nd and 3rd thoracic sympathetic ganglia,the sympathetic nerves at the lower limb acupoints originate from the thoracolumbar sympathetic trunk and the paravertebral ganglia from the 1st to the 3rd lumbar vertebrae,and the sympathetic nerves at the trunk acupoints obviously have local segmental characteristics,The sympathetic nerve fibers are all accompanied by their corresponding spinal nerves,which originate from the corresponding spinal lateral horn and thoracolumbar sympathetic ganglia.Conclusion:(1)Based on high-quality clinical evidence,it is found that the high-frequency acupoints for the treatment of post-stroke depression are Baihui,Taichong,Neiguan,Shenmen,Sanyinjiao,Yintang,Sishencong,Zusanli,shenting and Hegu;The core acupuncture prescriptions are Baihui,Taichong,Shenmen and Neiguan.(2)Based on the anatomical database of acupoints structure,the nerves commonly distributed at acupoints for the treatment of post-stroke depression have segmental characteristics.The nerves conduction distributed at acupoints on the head and face originate from the trigeminal nerve and cervical plexus,the nerves conduction distributed at acupoints on the upper limbs originate from the brachial plexus,the nerves conduction distributed at acupoints on the lower limbs originate from the lumbar plexus and sacral plexus,and the nerves conduction distributed at acupoints on the trunk originate from the spinal nerve of the corresponding segments.Therefore,acupuncture treatment of post-stroke depression may stimulate the trigeminal nerve and spinal nerve,transmit acupuncture signals to the central nervous system and regulate brain function,which is a possible neural transmission mechanism for the treatment of post-stroke depression. |
