Clinical Observation Of "Tiao Fu Tong Luo " Therapy In Children With Spastic Cerebral Palsy Complicated With Cognitive Impairment

Objective:Through the study of Gesell scale and modified Ashworth spasm classification to observe the influence of "Tiao Fu Tong Luo " on developmental quotient,development level of each ergoregion and spasm level before and after treatment of children with spastic cerebral palsy with cognitive impairment and the relationship between different treatment periods and clinical efficacy,so as to provide a theoretical basis for the selection and adjustment of clinical treatment plan and treatment period in the future.Content and Methods:64 children with spastic cerebral palsy complicated with cognitive impairment were divided into observation group and control group by random number method,32 cases in each group.The control group was treated with functional training(40min)+repetitive transcranial magnetic stimulation(30min),and the observation group was additionally treated with "Tiao Fu Tong Luo "(massage 40 min,scalp acupuncture 30min).Both groups were treated once a day,5 times a week,and rested for 2 days for a total of 3 months.Using Gesell scale and modified Ashworth spasm scale as evaluation indexes,the developmental quotient of Gesell’s functional areas and muscle tension of biceps femoris and triceps calf muscles were evaluated before treatment,1.5 months after treatment and after treatment,respectively.Results:1 In this study,3 patients fell off due to insufficient course of treatment and 1 patient fell off due to adding medicine in the middle,A total of 60 children were eventually included,including 30 cases in each group.Before treatment,there were no statistically significant differences in gender,age,disease,Gesell’s developmental quotient and modified Ashworth spasm grading score between the two groups(P > 0.05),which was comparable.2 Gesell result analysis: DQ of 5 major energy regions in the two groups after 1.5months and 3 months of treatment were compared within the group,and all of them were improved and statistically significant(P < 0.05).In terms of adaptive development quotient,there was no significant difference between the two groups after 1.5 months of treatment(P >0.05).After 3 months of treatment,the difference between the two groups was statistically significant(P < 0.05).In terms of overall development level,after 1.5 months of treatment,there was no statistical difference between the two groups in the comparison of developmental quotient in the five major energy regions(P > 0.05).After 3 months of treatment,there were statistically significant differences in language development quotient,gross motor development quotient and fine motor development quotient between the two groups(P <0.05),while there were no statistically significant differences between individual and social development quotient,indicating that the observation group was superior to the control group in improving the overall development level except for the individual and social energy area.3 Results analysis of modified Ashworth muscle tension grading: In the comparison results of the scale,the biceps femoris muscle tension and triceps calf muscle tension were evaluated within the two groups after 1.5 months and 3 months of treatment respectively,and the differences were statistically significant(P < 0.05).After 1.5 months and 3 months of treatment,biceps femoris and triceps calf muscle tension grading were compared between groups,and the differences were statistically significant(P < 0.05),indicating that the observation group was superior to the control group in improving muscle tension grading score.Conclusion:1 Combined with functional training and r TMS,"Tiao Fu Tong Luo " therapy can effectively improve the adaptability,language and motor development level of children with spastic cerebral palsy complicated with cognitive impairment,and reduce the degree of spasm,with better efficacy than the control group.However,it has a certain improvement effect on individuals and social energy areas,but there is no significant difference compared with the control group.2 In the choice of treatment period,the clinical effect of 3 months treatment is obviously better than 1.5 months treatment.
