| Objective: To analyze the clinical characteristics of patients with pericardial effusion in a tertiary hospital in Shihezi,Xinjiang,and provide some references for the clinical diagnosis and treatment of this disease in the future.Methods: Logged into the electronic medical record system to collect a total of 1080 patients with pericardial effusion diagnosed in the First Affiliated Hospital of Shihezi University School of medicine from September 1,2015 to March 31,2019,listed according to ICD-10 codes.A total of 1047 patients with pericardial effusion were studied according to the inclusion/exclusion criteria.According to the classification criteria of pericardial effusion proposed by the 2015 ESC pericardial disease diagnosis and treatment guidelines,the fluid volumes of the research subjects were divided into a micro small amount group,a medium amount group,and a large amount group.Gender subgroups;According to the different age strata,they were divided into the young and middle-aged group(18–64 years old)and the old group(over 65 years old).The differences of the constituent ratios of the primary diseases of pericardial effusion in different effusion volume groups,different gender and different age groups were compared.Data in the study were entered into IBM SPSS 26.0 software for analysis.Result: A total of 1047 patients with pericardial effusion were included in the study.1.Basic information: The average age of the subjects is 45.8±4.6 years old.There were 589 males(56.3%)and 458 females(43.7%).There were 557 cases(53.2%)in the small group,362 cases(34.6%)in the medium group and 128 cases(12.2%)in the large group.There were 521 cases(49.8%)in the young and middle-aged group(18-64 years old)and 526 cases(50.2%)in the elderly group(over 65 years old).2.The order of formation of the primary disease of pericardial effusion is as follows: malignant tumor cases(267 cases,25.5%),tuberculous pericarditis cases(223 cases,21.3%),heart failure cases(201 cases,19.2%),hematological diseases(34 cases,3.2%),infection cases(28 cases,2.7%),Finally,272 patients(26.1%)had unclear primary disease.3.Comparison of primary disease constitution in different effusion volume groups: Patients with neoplastic,tuberculosis,heart failure,infectious and unexplained pericardial effusion had significantly statistical differences in the composition of the different effusion volume groups(P < 0.05).The proportion of primary disease as malignant tumor in the medium group among the three groups was significantly higher than that in the other two groups(P < 0.05).Among the three groups,the proportion of the primary disease of tuberculosis in the large group was from high to low,followed by the large group,the medium group,and the micro small group(P < 0.05).The proportion of the primary disease of heart failure in the large group was significantly lower than that in the medium group and the small group(P < 0.05).The proportions of patients with pericardial effusion with unclear primary disease in the micro-small amount group were,from high to low,micro-small amount group,medium amount group and large amount group(P < 0.05).There was no significant difference in the constituent ratios of other etiologies among the three groups.4.Comparison of primary disease constitution in different sex groups: The proportion of female patients with pericardial effusion primary diseases including heart failure,immune system diseases,hypothyroidism and unexplained diseases was significantly higher than that of male patients with pericardial effusion,and the proportion of tuberculosis pericardial effusion was significantly lower than that of male patients with pericardial effusion(P < 0.05).5.Comparison of primary disease constitution in different age groups: There were significant differences in composition ratios of patients between the two groups whose primary diseases were malignant tumor,heart failure,hypothyroidism and unexplained pericardial effusion.The proportion of primary diseases including malignant tumor and heart failure in the aged group was significantly higher than that in the young and middle-aged group(P < 0.05).Malignant pericardial effusion accounted for the highest proportion in the elderly group.The proportion of patients with no clear cause in the middle-aged and young group was significantly higher than that in the elderly group(P < 0.05).Conclusion:1.The proportion of patients with pericardial effusion whose primary disease is malignant tumor in our hospital is the highest,and the proportion of patients with tuberculous pericardial effusion is the second.2.The primary diseases of patients with pericardial effusion with different effusion volumes are different.For patients with moderate to large pericardial effusion,the primary diseases are mainly tuberculosis and malignant tumor.3.The primary diseases of elderly patients with pericardial effusion are mostly malignant tumors. |
