Objective:Obesity has become a public health problem throughout the world,and the incidence of childhood obesity is also on the rise.Harm from the obese adolescents is increasingly recognized,and obesity in adolescence also affects the incidence of various diseases in adulthood.Therefore,the intervention and treatment of childhood obesity is very important.At present,the treatment of childhood obesity has the characteristics of long cycle and insignificant treatment effect.Researchers are also exploring and seeking more effective methods to treat obesity.Studies have shown that obesity can lead to abnormal glucose and lipid metabolism and changes in thyroid hormones.The level of vitamin D in obese children is low.However,the relationship between vitamin D levels and glucose and lipid metabolism and thyroid hormone levels in obese children is still unclear.The purpose of this study was to analyze the correlation between the vitamin D level,glucose and lipid metabolism,and thyroid hormone level in obese children,so as to provide a scientific basis for clinical intervention treatment of obese children.Methods:1.From March 2019 to February 2022,109 obese children(70 males and 39females)who were treated in the Child Health Clinic of Liuzhou Maternal and Child Health Hospital were selected as the obesity group,and during the same period,104 healthy non-obese children(62 males and 42 females)who underwent physical examination in the Child Health Clinic of the Children’s Health Department were selected as the healthy control group.2.General data such as the age,height,weight,waist circumference,hip circumference,body mass index(BMI)of the two groups was Collected,and the serum levels of 25-hydroxyvitamin D(25(OH)D),Total Cholesterol(TC),Triglycerides(TG),High Density Lipoprotein(HDL),Low Density Lipoprotein(LDL),Triiodothyronine(T3),Thyroxine(T4),Free Triiodothyronine(FT3),Free Thyroxine(FT4),Thyroid-stimulating hormone(TSH)in the two groups were detected.The serum fasting blood glucose(FBG)and fasting insulin(FINS)of the obesity group were detected,and the insulin resistance index(HOMA-IR)was calculated.3.The differences of the general data and serum indexes in the two groups were compared,and the correlations between vitamin D level,abnormal glucose and lipid metabolism and thyroid function in the obese children were analyzed.Results:1.The BMI and Waist-to-Hip Ratio(WHR)of the obesity group were higher than those of the healthy control group;The serum 25(OH)D level in the obesity group was statistically lower than that in the healthy control group;The blood lipid indexes TC,TG,and LDL in the obesity group were higher than those in the healthy control group,while HDL was statistically lower than that in the healthy control group;In the obesity group,the levels of thyroid hormones T3,FT3,and TSH were significantly higher than those in the healthy control group,while the level of FT4 was lower than that in the healthy control group.2.In the obesity group,there were significant differences in the levels of Vit D,TC,TG,LDL,TSH,and HOMA-IR among the three groups of mild obesity,moderate obesity and severe obesity.The level of Vit D decreased with the increase of obesity and it was negatively correlated with BMI and WHR.TC,TG,LDL,TSH,and HOMA-IR increased with the increase of obesity degree.TSH and HOMA-IR were positively correlated with BMI.3.Compared with the vitamin D deficiency group,the insufficiency group and the sufficient group,the BMI and WHR decreased with the increase of Vit D levels;The levels of TC and TG decreased with the increase of Vit D levels and they were negatively correlated with Vit D;TSH levels and HOMA-IR decreased with the increase of Vit D levels,TSH and Vit D levels were negatively correlated.The proportion of insulin resistance in the Vit D insufficiency and deficiency group was significantly higher than that in the Vit D sufficient group.4.TSH levels were divided into groups,and Vit D level decreased with the increase of TSH level.5.Compared with obese children without IR,obese children with IR had higher levels of BMI,WHR,TG and TSH,and lower levels of Vit D and HDL.HOMA-IR was positively correlated with BMI,WHR and TG,and negatively correlated with HDL.Conclusion:Obese children have low levels of Vit D,and have different degrees of abnormal glucose and lipid metabolism and changes in thyroid hormones.Vit D levels in obese children were negatively correlated with TC,TG,and TSH.Obese children with lower Vit D levels were more likely to develop IR,while obese children with IR showed higher levels of TSH.This study not only reflects the correlation between various complications in obese children,but also provides a scientific basis for finding new treatment methods for obese children.
