Analysis Of Laboratory-related Indicators Of ABO Hemolysis In Children With Thalassemia Gene

Abstract/Summary:
Objective: To investigate the differences of total bilirubin(TSB),hemoglobin(Hb),mean erythrocyte hemoglobin content(MCH)and the three tests of hemolysis results in ABO-HDN children who with or without thalassemia gene,it may provide a reference for clinical treatment of ABOHDN children with thalassemia gene.Methods: Collecting the heel blood of the children within 7 days to screen thalassemia and registering the clinical data of the children.The children with maternal-infant blood group incompatibility were tested by three hemolysis tests.The ABO-HDN children were divided into those with and without thalassemia groups.The differences between two groups in TSB,Hb,MCH,direct antiglobulin test(DAT),free antibody test,and positive rate of all three hemolysis tests were analyzed statistically.Results:1.A total of 1298 positive cases with a positive screen for thalassemia were collected in this study,and 52 cases with ABO-HDN were selected as the thalassemia carrier group.A total of 141 ABO-HDN children without thalassemia were selected as the non-thalassemia group.There were significant differences in TSB among 193 ABO-HDN children in gestational age,birth weight and age at diagnosis.The older the patient was,the more serious the hemolysis was.2.The results of three hemolysis tests in 193 ABO-HDN children showed that the TSB and Hb levels of DAT-positive group and DAT-negative group were [235.7(90.5)VS 221.1(74.6)]μmol/L and [(146.58±27.32)VS(168.54 ± 19.51)]g/L,respectively.The difference was statistically significant.The DAT-positive group had a higher TSB level and a lower Hb level,indicating that the DAT-positive children had more severe hemolysis and anemia.3.54 children were positive in all three hemolysis tests,and 139 children were negative.The TSB and Hb levels of the two groups were [243.4(97.4)VS 221.2(75.0)]μmol/L and [(146.06±25.73)VS(167.80±20.98)]g/L,respectively.The difference was statistically significant.Children with all positive of three hemolysis tests have higher TSB levels and lower Hb levels,indicating that the children with all positive of three tests of hemolysis have serious hemolysis and anemia.4.Among ABO-HDN children,there was 52 children with thalassemia and 141 children without thalassemia.In this two groups,the TSB[229.9(82.1)VS204.9(76.8)]μmol/L,Hb[(153.92±20.89)VS(164.59±25.02)]g/L and MCH[(32.33±3.46)VS(36.26±1.80)] pg levels were statistically different.The TSB of ABO-HDN children with thalassemia was higher,and Hb and MCH were lower,indicating that the hemolysis and anemia of ABO-HDN children with thalassemia gene were more serious than those without thalassemia.Conclusion:1.The older the children,the more severe the hemolysis is when ABOHDN occurs.DAT positivity is a good indicator to predict the increase of serum bilirubin level,and the hemolysis and anemia are more severe in DAT positive children.2.Children with positive results of all three hemolysis tests have more severe hemolysis and anemia,and the three hemolysis tests can have a good indication effect on the hemolysis and the severity of the disease.3.Among ABO-HDN children,who carried thalassemia gene had higher TSB,lower Hb and MCH,and more severe hemolysis and anemia when ABO-HDN occurred.
