Abstract/Summary:
ObjectiveTo analyze the correlation of fetal birth weight and macrosomia with postpartum stress urinary incontinence(SUI)at different parities.MethodsA total of 1049 patients who underwent pelvic floor ultrasonography from June 2018 to January 2021 were selected as the research subjects.The patient’s basic information,history of underlying diseases,delivery-related information,postpartum-related information,and pelvic floor ultrasound examination results were collected.Using R software and Empower(R)software,the influencing factors of SUI were analyzed,and the multivariate regression analysis model was used to adjust the confounding factors to analyze the correlation between fetal birth weight and macrosomia and maternal occurrence,and further use the threshold saturation effect model to explore the fetal birth weight.Thresholds for the risk of postpartum SUI with increased birth weight.Results1.Among the 449 primiparas in this study,SUI occurred in 65 cases,with an incidence rate of 14.48%;among the 450 multiparous women,SUI occurred in 60 cases,with an incidence rate of 13.33%.2.After adjusting for confounding factors,the results of multiple regression analysis showed that fetal birth weight was associated with the occurrence of SUI in primiparas(OR=2.24 95%CI:1.07~-4.68,P=0.0315).Compared with primiparas who delivered fetuses weighing less than 4 kg,primiparas who delivered macrosomia(≥4 kg)had a 3.11-fold increased risk of SUI after delivery(OR=4.11,95%CI:1.01-16.70,P=0.046).3.Regardless of whether confounding factors were adjusted,there was no correlation between fetal birth weight,macrosomia and the occurrence of postpartum SUI in multiparous women(P>0.05).4.After adjusting for variables such as age,mode of delivery,maternal BMI,and gestational age,the threshold saturation effect model was used,and the result showed that the threshold for fetal birth weight was 2.63 kg.Furtherly,the multiple regression analysis showed that,when the birth weight of the fetus was less than 2.63kg,there was no significant relationship with the occurrence of postpartum SUI in primiparas(OR=0.32,95%CI:0.00-70.64,P=0.6763).When the birth weight of the fetus was greater than 2.63 kg,birth weight of the fetus was associated with the occurrence of postpartum SUI in primiparas(OR=2.45,95%CI:1.13-5.28,P=0.0225).Conclusion1.There is a correlation between fetal birth weight and postpartum SUI in primiparas:When the fetus gains 1kg,the risk of SUI in primiparas increases by 1.24 times;especially,when the birth weight of the fetus exceeds 4kg,the risk of SUI in primiparas will increase 3.11 times.2.When the birth weight of the fetus in primipara is more than 2.63kg,the risk of postpartum SUI will increase significantly.3.There was no correlation between the birth weight of the fetuses of multiparous women and the occurrence of postpartum SUI;the risk of postpartum SUI of multiparous women who delivered macrosomia(≥4 kg)increased by 8%,but the difference was not statistically significant.4.Whether it is the first delivery or the second delivery,The mode of delivery and the formation of the internal urethral funnel are closely related to the occurrence of SUI.Vaginal delivery or people with urethral funnel should pay special attention to prevent the occurrence of SUI.
