Objective:To observe and analyze the clinical efficacy of different proportions of Bingxiao Powder in the treatment of preoperative limb swelling and pain in patients with ankle fractures.Methods:Ninety patients who met the diagnostic criteria in Shandong Hospital of traditional Chinese Medicine from October 2021 to September 2022 were randomly divided into three groups:group A(n=30),group B(n=30)and control group(n=30).After admission,all patients underwent routine preoperative examination,low molecular weight heparin was subcutaneously injected according to the corresponding dose of body weight to prevent deep venous thrombosis of lower extremities,once a day;20%mannitol solution was given intravenously to promote swelling reduction according to the corresponding dose of body weight,once a day.Guide patients to raise the affected limb about 20-30 degrees after bed rest,which is beneficial to the venous reflux of the affected limb and promote the regression of swelling.The patients in the control group were treated with the above basic treatment.The patients in the treatment group were treated with basic treatment combined with external application of Bing Nitro Powder.Bingniangsan was provided by the traditional Chinese medicine store in the hospital.in group A,Bingniangsan was mixed evenly according to the proportion of borneol 10g and mirabilite 1000g,put it into a cloth bag with five strip pockets,put the bag close to the ankle and apply it externally for 24 hours.If the ice nitrate powder is hardened due to water absorption after being used for a period of time,the ice nitrate powder is replaced,and the used ice nitrate powder is poured out in the shade to dry and recycled.Group B was mixed with borneol 20g and mirabilite 1000g,and the rest was the same as group A.The ankle swelling and pain score of the affected side were measured at 9:00 a.M.in all patients,and the treatment lasted for 3 days.The timing of operation was judged by swelling score,and the preoperative preparation time was calculated after reaching the standard of operation.The collected data were statistically analyzed by SPSS26.0,and different kinds of data were analyzed by appropriate statistical methods to evaluate the curative effect of the three methods on preoperative swelling and pain of ankle fracture.Results:1.Comparison of swelling value of affected limb:comparison between groups:during the 3-day treatment,group An and B had obvious advantages in reducing swelling compared with the control group,and both had statistical significance(P<0.05).There was no significant difference in detumescence effect between group An and group B(P>0.05).Intra-group comparison:all the patients in the three groups had the effect of reducing swelling within 3 days of treatment,which was statistically significant(P<0.05).two。.Comparison of the detumescence rate of affected limb:compared with the control group,the detumescence rate of group An and B was significantly higher after 3 days of treatment(P<0.05),but there was no significant difference between group An and group B(P>0.05).3.Comparison of preoperative preparation time:compared with the control group,the preoperative preparation time in group An and group B was significantly shorter than that in the control group(P<0.05).There was no significant difference in preoperative preparation time between group An and group B(P>0.05).Compared with the control group,the preparation time before operation in group An and group B was significantly reduced.4.Comparison of pain score:comparison between groups:one day after treatment,there was no significant difference in pain score between group An and group B(P<0.05).Compared with the control group,the pain score in group A was significantly lower than that in group B(P<0.05).The pain score in group B was significantly lower than that in the control group(P<0.05).After 2 and 3 days of treatment,there were significant differences in pain scores among group A,group B and control group(P<0.05).Intra-group comparison:the pain scores of the three groups after treatment were significantly lower than those before treatment,and the differences were statistically significant(P<0.05).There were significant differences in the pain scores of each group on the 1st,2nd and 3rd day of treatment(P<0.05),that is,the three treatments could reduce the pain of patients to some extent.There was no significant difference in pain relief between group An and group B after 1 day treatment,but there was significant difference 2-3 days after treatment(P<0.05).Conclusion:Bingxiao Powder promotes the regression of edema after fracture,reduces the degree of pain of patients,shortens the preoperative preparation time of patients,and lays a solid foundation for later surgical treatment.At the same time,the high proportion of borneol powder has a good analgesic effect in clinical trials,which has a high clinical application value in the treatment of preoperative limb swelling and pain in patients with ankle fracture,and gives full play to the advantages of traditional Chinese medicine treatment.it is worth popularizing and applying in clinic.
