Effect Of Circular Pressure Cooling Therapy On Perioperative Soft Tissue Swelling And Pain In Patients With Ankle Fracture

Objective: To observe the effect of different intervention measures in the treatment of perioperative soft tissue swelling and pain of patients with ankle fracture by using conventional ice compress method and circulatory pressure cooling therapy through experimental study,and to provide new intervention methods and ideas for the future nursing and clinical work of ankle fracture.Methods: A total of 70 patients with ankle fractures admitted to the trauma center of a third class hospital in Hebei province from October 2018 to October 2019 were collected as subjects.Patients were randomly divided into experimental group and control group by the method of drawing lots in simple random sampling before surgery,with 35 cases in each group.On the basis of routine orthopedic care,patients in the test group were treated with circular pressure cold treatment device for swelling and pain,while patients in the control group were treated with conventional ice packs.The application time of cold therapy in both groups was 20min/time,with an interval of 4h between each time.The researchers used Numerical Rating Scale(NRS)to assess the pain of patients,and recorded the pain scores before and after the intervention at 24 h,48h and 72 h.Figure-of-eight-20 method was used to measure the swelling of the fixed position of the ankle joint(medial and lateral malleolus and middle circumference diameter of the foot),and then to evaluate the degree of the swelling of the affected limb.The incidence and waiting time of tension blisters at the fracture site were compared between the two groups.Will Open reduction and internal fixation(Open reduction and internal fixation,ORIF)of 70 patients,using the random number table method will be divided into three groups: application within 48 h of postoperative circulation pressure cold therapy device of experimental group(n=24);The control group(n=23)who were treated with circulating pressure cold therapy device within 24 hours after the operation.The blank group(n=23)who only received routine care after fracture surgery and did not receive any cold treatment.The experimental group and the control group were given cold treatment every 4h starting from 2h after the operation,and the pain scores of the subjects before and after the intervention for 24 h and 48 h were recorded and compared.Results: There was no statistically significant difference between the two groups in the basic data of patients before surgery and the NRS score and swelling before intervention(P>0.05).Longer duration of intervention,two groups of patients with limb swelling value,NRS score all showed a trend of decrease,between the groups(F=4.095,P=0.047;F=34.852,P=0.000),between the point(F=1023.497,P=0.000;F=347.327,P=0.000),between groups,interaction between point(F=179.340,P=0.000;F=23.470,P=0.000)difference had statistical significance(P<0.05),and swelling value,NRS score group patients were lower than the control group;There were statistically significant differences in the incidence and waiting time of tension vesicles between the two groups(P<0.05).There was no statistically significant difference in the basic data and NRS score before intervention in the three groups(P>0.05),suggesting comparability.After 24 h and 48 h of intervention,the difference between the blank group and the control group and the experimental group was statistically significant(P<0.05),and the difference between the experimental group and the control group was not statistically significant(P>0.05).The NRS score of the experimental group and the control group at each time point was lower than that of the blank group,and the difference was statistically significant(P<0.05).There was no statistical difference in NRS score between the experimental group and the control group(P>0.05).Conclusion: The early application of circulatory pressure cold treatment device in patients with ankle fracture is helpful to reduce the pain and soft tissue swelling,reduce the incidence of tension blisters,and shorten the waiting time before surgery.The application of circulatory pressure cooling device after surgery can improve the pain perception of patients,facilitate early postoperative rehabilitation exercise,and accelerate the application of rehabilitation surgery in the treatment of ankle fracture.This therapy has low adverse reaction,high clinical satisfaction,simple operation,easy fixation and obvious effect,which is worth popularizing and applying in orthopedic nursing clinic.
