Discussion On The Medication Rule Of Professor Liu Ruixia In The Treatment Of Thyroid Nodules With Anxiety And Depression

Objective: Based on Traditional Chinese Medicine Inheritance Computing Platform System(V3.0),the medical records of Professor Liu Ruixia’s treatment of thyroid nodule accompanied by anxiety and depression were studied.The analysis includes the general situation of patients,treatment rules and medication rules.Therefore,learning and inheriting Professor Liu Ruixia’s experience in treating this disease provides a new idea for better treatment of thyroid nodule accompanied by anxiety and depression.Methods: The medical records of patients diagnosed with thyroid nodule accompanied by anxiety and depression at the outpatient department of Professor Liu Ruixia,Affiliated Hospital of Shandong University of Traditional Chinese Medicine from June 1,2019 to December 1,2022 were collected.The data were analyzed with the help of Traditional Chinese Medicine inheritance computing Platform(V3.0),including patient gender,age of onset,symptoms,syndrome types,tongue and pulse analysis.As well as the analysis of treatment rules and methods,the author made an analysis of drug flavor,drug frequency,drug association and drug cluster for the prescriptions prescribed by Professor Liu Ruixia,and further analyzed the core drug pairs and core prescriptions,thus summarizing the overall medication rule of Professor Liu Ruixia in the treatment of thyroid nodule accompanied by anxiety and depression.Results:The incidence rate of thyroid nodules with anxiety and depression increased from the age of 31 to 40 years old,peaked at the age of 51 to 60 years old,and the incidence rate of thyroid nodules with anxiety and depression decreased significantly at the age of 61 to 70 years old.There were fewer patients over 70 years old,and the majority of patients with this disease were women.The main symptoms of thyroid nodules with anxiety and depression are irritability,poor sleep,foreign body sensation in the neck,dizziness,and neck distension,etc.,and irritability often occurs at the same time with poor sleep,foreign body sensation in the neck,dizziness,and other symptoms;It was found that the patients were mostly dark red tongue,red tongue,light red tongue and purple dark tongue,and the tongue coating was mainly white greasy coating,yellow greasy coating,thin white coating and thin yellow coating;The pulse is mainly composed of smooth pulse,followed by deep pulse,and the occurrence frequency of slow pulse,thin pulse and astringent pulse is relatively small;The analysis of syndrome types showed that the thyroid nodules with anxiety and depression were mainly liver depression and spleen deficiency syndrome,liver depression and qi stagnation syndrome,and qi stagnation and phlegm coagulation syndrome.Professor Liu Ruixia treats thyroid nodules with anxiety and depression mainly by soothing the liver and resolving depression,resolving phlegm and dispersing nodules,soothing the liver and strengthening the spleen,regulating qi and activating blood flow,and at the same time,with the treatment of invigorating the spleen and replenishing qi,regulating qi and resolving phlegm,clearing the liver and purging fire;Through the analysis of Professor Liu Ruixia’s medication rules for the treatment of this disease,it was found that a total of 230 drugs were involved in 116 medical cases,which could be divided into 17 categories,with a total frequency of 1915;The drugs mainly include drugs for tonifying deficiency,clearing heat,activating blood circulation and removing stasis,relieving cough,resolving phlegm and asthma,promoting water and moisture,and regulating qi;The analysis of the four gases and five flavors of drugs found that the use of cold drugs and warm drugs was more,and the drugs were mostly sweet and bitter;The meridians of drugs are concentrated in the liver,spleen and lung meridians;According to the analysis of the practical frequency of drugs,the top eight drugs are licorice,prunella vulgaris,angelica sinensis,fructus aurantii,ligusticum chuanxiong and zedoary Rhizoma atractylodis,Radix paeoniae rubra and Chinese yam,among which Prunella vulgaris-Angelica sinensis,Glycyrrhiza uralensis-Angelica sinensis,Angelica sinensis-Fructus aurantii are used more than 50 times,which is a high-frequency drug combination;The drug association analysis found that red peony-scallop-fennel-fructus aurantii,fructus aurantii-red peony-fritillaria-fennel,yam-fennel-fructus aurantii were the top three drug combinations.Based on the k-means algorithm and regression simulation analysis,the core formula rule is obtained,and three core combinations are obtained,as follows: licorice,prunella vulgaris,fructus aurantii,angelica sinensis,and red peony,with the frequency of use of 60 times;Poria cocos,Bupleurum chinense,Angelica sinensis,Prunella vulgaris,Fritillaria thunbergii,used31 times;Liquorice,Angelica,Ligusticum chuanxiong,Prunella vulgaris,Angelica dahurica,used 25 times.Through the above research,the overall medication rule of Professor Liu Ruixia in the treatment of thyroid nodules with anxiety and depression is summarized,which provides reliable clinical basis for the treatment of this disease.Conclusion: Based on the statistical analysis of the Traditional Chinese Medicine Inheritance Computing Platform(V3.0),Professor Liu Ruixia combined disease differentiation-syndrome differentiation-constitution differentiation" in the treatment of thyroid nodules with anxiety and depression,while regulating the qi mechanism,paying attention to the rational use of promoting blood circulation and removing stasis,and playing the role of relieving depression and removing gall.
