Based On The Clinical Database To Explore The Medication Rule Of Professor Sun Bing In The Treatment Of Polycytic Ovary Syndrome During Follicular Phase

Abstract/Summary:
ObjectiveBased on the auxiliary platform of traditional Chinese medicineinheritance(V2.5)and online pharmacology,the clinical medical record data of Professor Sun Bing in the treatment of polycystic ovarian syndrome follicular phase were processed and statistically analyzed,and the drug use rules of Professor Sun Bing in the treatment of polycystic ovarian syndrome follicular phase were summarized,the core prescription was explored,the potential mechanism of the core prescription was explored,and the experience of Professor Sun Bing in the treatment of polycystic ovarian syndrome was inherited.MethodStudy 1:Extract the outpatient medical record data of Professor Sun Bing in treating patients with polycystic ovary syndrome,and use the software of traditional Chinese medicine inheritance assistant platform(V2.5),SPSS Modeler 18,SPSS Statistics 25,etc.to make statistics on the four diagnostic data and clinical effectiveness of patients,summarize the characteristics and taste of drugs,the course,the frequency of application,and drug pair combination,clarify the core drugs,explore the core prescription,and explore the law of differentiation and treatment.Study 2:TCMSP database and UniProt database were used to collect and screen the active ingredients and targets of the core prescription,using DisGeNET,OMIM and GeneCards database to retrieve the relevant genes of polycystic ovary syndrome.Take the intersection of the core prescription and the target of disease,and build the network of "Chinese material-active-component-target-disease "through Cytascape3.9.Osoftware;Import intersection targets into STRING website to build PPI network and use Cytoscape 3.9.Osoftware to analyze the topological parameters of intersection targets to screen core targets;The DAVID database GO function enrichment analy sis and KEGG pathway enrichment analysis were applied to construct the "active-component-target-pathway" network.ResultsStudy 1:A total of 779 prescriptions were collected,including 215 patients,with main symptoms including long-term infertility and irregular menstruation.A retrospective analysis of 101 patients according to the Napai standard showed that Professor Sun Bing’s total effective rate in treating polycystic ovary syndrome was 84.16%,and the longer the treatment period for patients,the higher the effective rate.Patients were divided into two groups:the traditional Chinese medicine group and the combination of traditional Chinese and Western medicine group,and further analysis based on literature inclusion in the Western medicine group showed that the total effective rate of the traditional Chinese medicine group was higher than that of the Western medicine group(82.1%vs 72%),with no statistically significant difference(P>0.05).The effective rate of 86.7%(39/45)in the combination group was higher than 82.1%(46/56)in the pure Chinese medicine group and 72%(72/100)in the pure Western medicine group,with no statistically significant difference(P>0.05).Comparing the treatment courses of the traditional Chinese medicine group and the combination group with the Western medicine group included in the literature(72/100),the treatment courses of the traditional Chinese medicine group and the combination group were≥4(χ~2=4.082,P=0.043),≥ 3(χ~2=5.036,P=0.025),≥ 2(χ2=5.75,P=0.016),≥1(χ The effective rate of 2=4.344,P=0.037)was higher than that of the Western medicine group.According to the drug frequency,frequency division,and efficacy classification results,the prescriptions with higher frequency of use include Codonopsis pilosula,Angelica sinensis,Cuscuta chinensis,Ligustrum lucidum,Eclipta chinensis,roasted licorice,Atractylodes macrocephala,and Rehmannia glutinosa.The classification of drugs is mainly based on tonifying deficiency,promoting blood circulation and resolving stasis,and regulating qi.The medicinal properties are relatively warm and flat,with a taste that is sweet,bitter,and spicy.The meridians of the liver,spleen,and kidney are the most common.The drug dosage ranges from 6~45g,with 31 drug modes,mainly including Cuscuta chinensis,Codonopsis pilosula,Angelica sinensis,Eclipta chinensis,and Ligustrum lucidum.Cluster analysis mining of 4 drug combinations and 7 new prescriptions.Further association rules and cluster analysis were conducted to remove the traditional Chinese medicine licorice,resulting in 17 combinations of paired and angular herbs,4 commonly used cluster combinations,and 1 core prescription,including 12 traditional Chines e medicines including Codonopsis pilosula,Angelica sinensis,Cuscuta chinensis,Ligustrum lucidum,Eclipta chinensis,Atractylodes macrocephala,Morus parasiticus,Poria cocos,Liu Jinu,Huangqi,Huangjing,and Rehmannia glutinosa.Study 2:Through network pharmacology,73 active ingredients,254 targets and 138 intersection targets were obtained in the core prescription,and 19 compounds related to polycystic ovary syndrome were obtained in the core prescription,including quercetin,luteolin,naphthol,stigmasterol,7-methoxy-2-methylisoflavone,β-sitosterol,etc.T opological analysis and cluster analysis were carried out using PPI network and Cyt oscape software,and 12 core targets were selected,including MAPK1,TP53,CASP3,RB1,MAPK14,EGFR,MRC,EGR,ESR1,AKT1,FOS and STAT1.Cluster 1 consisted of 7 proteins of the seed gene MAPK1,and Cluster 2 consisted of 5 proteins of the seed gene EGR.GO analysis showed that the core targets involved drug response,positive regulation of gene expression,negative regulation of apoptosis,extracellular solution,polymer complex,enzyme binding,same protein binding,etc.KEGG enrichment analysis showed that the main signaling pathways of core targets included PI3K-AKT signaling pathway,AGE-RAGE signaling pathway,P53 signaling pathway,etc.ConclusionProfessor Sun Bing for the treatment of polycystic ovary syndrome clinical efficacy,can effectively improve the clinical symptoms of patients,promote follicular development,regulate the level of sex hormones.Professor Sun Bing’s treatment of polycystic ovary syndrome(PCOS)in follicular stage is based on Bazhen Erzhi decoction,which is composed of tonifying kidney and Yin,supplementing qi and blood,regulating qi and resolving phlegm,promoting blood circulation and removing blood stasis.The medicine is mainly used for tonifying deficiency,supplemented by qi-regulating and blood-activating drugs,with both Qi and blood in mind.2.It is speculated that the treatment of PCOS with core prescription is the result of intermolecular synergy,with the characteristics of multitarget,multicomponent,and multichannel.It provides theoretical basis for further experimental research and scientific support for clinical promotion of a safe,economic and effective treatment method.
