| Objective: Based on Real World Data of clinical,this study discusses the clinical efficacy of Jining tablets in the treatment of tachyarrhythmias,and observes the influencing factors of its combination and drug efficacy,which provides a certain reference for the clinical treatment of tachyarrhythmias.Methods: Based on Real World Study,a prospective observational study design was adopted,and 100 patients who met the inclusion criteria were included in the outpatient clinic of the Department of Cardiology of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from December 2021 to January 2023.In the process of observation,according to the clinical symptoms of TCM,patients were divided into four TCM symptom groups: Yin deficiency and fire group,qi and yin deficiency group,phlegm and fire disturbing group,and heart and spleen deficiency group;According to the patient’s combined medication,it is divided into8 drug combinations: Combination 1 refers to single use of Jining tablets;Combination 2 refers to the combination of western medicine based on the use of Jining tablets;Combination 3 refers to the combined use of traditional Chinese medicine on the basis of the use of Jining tablets;Combination IV refers to the use of traditional Chinese patent medicines and simple preparations on the basis of the use of Jining tablets;Combination 5 refers to the combination of western medicine and traditional Chinese medicine on the basis of the use of Jining tablets;Combination 6refers to the combination of western medicine and traditional Chinese patent medicines and simple preparations based on the use of Jining tablets;Combination 7 refers to the combination of traditional Chinese medicine and traditional Chinese patent medicines and simple preparations based on the use of Jining tablets;Combination 8 refers to the combination of western medicine,traditional Chinese medicine and traditional Chinese patent medicines and simple preparations based on the use of Jining tablets.Through the collection of clinical data,the clinical efficacy and safety of patients before and after taking the drug were evaluated,and the efficacy differences between the four groups of medical evidence types and the efficacy differences between the eight drug combinations were analyzed,so as to explore the best TCM symptoms and the best drug combination method corresponding to the treatment of Jining tablets,and analyze the influencing factors of drug efficacy.Results: 1.In this study,the age of patients with tachyarrhythmia who took Jining tablets was concentrated in 50-59 years old and 60-69 years old,and the TCM symptom type was mainly Yin deficiency and fire type.2.Compared with before treatment,the TCM syndrome score and Holter index decreased significantly(P<0.05).After stratified by age and sex,the effective number of TCM syndrome integral and Dynamic Electrocardiogram indicators were compared between groups(P>0.05),with no statistical significance.The effective number of TCM syndrome score and Dynamic Electrocardiogram indicators were compared between the four TCM syndrome groups,and there was no statistically significant difference between the TCM syndrome score,atrial fibrillation,supraventricular tachycardia and ventricular tachycardia(P>0.05).There was statistically significant difference between the groups of atrial premature beats and ventricular premature beats(P<0.05).Follow-up multiple comparisons were carried out by Bonferroni method.The inefficiency(5%)of the yin deficiency and fire hyperactivity group was lower than that of the qi and yin deficiency group(14%),the phlegm and fire disturbance group(40%),and the heart and spleen deficiency group(50%)in patients with ventricular premature beats,with statistically significant differences(P<0.05).The inefficiency(100%)of the heart and spleen deficiency group in patients with atrial premature beats was higher than that of the yin deficiency and fire hyperactivity group(7%),the qi and yin deficiency group(20%),and the phlegm-fire disturbance group(50%),with statistically significant differences(P<0.05).3.According to the drug combination of the included cases,they were divided into 8 combinations.After collecting data,the number of effective cases of the 8 drug combinations was obtained,and the chi-square comparison of the effective rate between groups was not statistically significant(P>0.05),and the difference between groups was not significant.4.Logistic regression analysis was carried out on the factors that might affect the drug efficacy,and the effective TCM syndrome score was taken as the dependent variable.It was found that BMI,alcohol consumption and the clinical efficacy of drugs were negatively correlated,with statistical significance(P<0.05);Taking the effectiveness of Dynamic Electrocardiogram as the dependent variable,it was found that smoking and less sleep time were negatively correlated with the clinical efficacy of drugs,with statistical significance(P<0.05).Conclusion: In this study,Jining tablets has obvious efficacy in improving TCM syndrome scores and Dynamic Electrocardiogram indicators in patients with tachyarrhythmia.Age and sex have no effect on the efficacy of Jining tablets in treating patients with tachyarrhythmia.In the treatment of ventricular premature beats with Jining tablets,the effect of yin deficiency and fire hyperactivity group is better than that of other syndrome groups;In the treatment of patients with atrial premature beats,the effect of the heart and spleen deficiency group is worse than that of other syndrome groups.The effect of Jining tablets combined with western medicine or traditional Chinese medicine or traditional Chinese patent medicines and other drug combinations was not better than that of Jining tablets alone.BMI,alcohol consumption,smoking,and low sleep duration are relevant factors that may affect the clinical efficacy of Jining tablets in the treatment of tachyarrhythmias. |
