| Objective:On the basis of Professor Wang Qi’s Nine Constitution Theory and Tongue Picture Theory of TCM,constitution and tongue image distribution in Type 2 Diabetes Mellitus(T2DM)and Metabolic associated fatty liver disease(MAFLD)were investigated.To study the relationship between the features of the tongue and the major structural types of MAFLD in patients with T2DM,the aim is to provide a new diagnostic and therapeutic approach for T2DM complicated by MAFLD.Methods:In this study,a total of 219 cases of T2DM accompanied by MAFLD were selected from Affiliated Hospital of Shandong University.General data,medical history data and tongue image were collected,and the constitution type of patients was determined by questionnaire.The database was set up to describe MAFLD patients’gender,age,Body Mass Index(BMI),Traditional Chinese Medicine(TCM),Frequency and Composition Ratio of Tongue Picture.Using SPSS 25.0 software,the goodness of fitχ~2 test,Pearson’sχ~2test,Fisher’s precise probability and correspondence analysis were performed,to investigate the composition of TCM and its relationship with tongue image in T2DM with MAFLD.Results:(1)There was no significant difference in gender(p>0.05),but there was significant difference in age group and BMI group(p<0.01).Among 219 patients with T2DM and MAFLD,110 were male and 109 were female,the ratio of male to female was about 1:1.The age range is from 22 to 81 years old,the number of the elderly group is the largest,110 people,overweight patients are the main component,accounting for 50.2%.(2)The distribution of TCM Constitution in MAFLD patients with T2DM was as follows:deficiency of Qi in 48 cases(21.9%)>phlegm-dampness in 40 cases(18.3%)>dampness-heat in 37 cases(16.9%)=blood stasis in 37 cases(16.9%)>deficiency of yin in 26 cases(11.9%)>stagnation of Qi in 18 cases(8.2%)>peaceful constitution in 8 cases(3.7%)>deficiency of yang in 5 cases(2.3%)>special constitution in 0 cases(0%).(3)There was no correlation between the main types of physical fitness and age(p>0.05),but there was correlation between the main types of physical fitness and gender and BMI(p<0.05).The majority of men had phlegm,qi,and dampness,while the women had qi deficiency and blood stasis.The percentage of Qi deficiency in overweight patients was the highest,and that of obese patients was the most.(4)The distribution of tongue image in MAFLD patients with T2DM was as follows:Among the color of the tongue,the most frequent occurrence was pale red tongue and red tongue,which occupied 24.7%and 19.6%,respectively.Light white tongue,dark red tongue,deep red tongue,pale purple tongue,purple tongue,ecchymosis tongue and deep purple tongue.In terms of the distribution of the tongue,the teeth and the fat tongue were the most important,accounting for 25.6%and 23.3%,respectively.Next,the shape of the tongue,the thin tongue,the tender tongue,the aged tongue,the pierced tongue and the cracked tongue.In the distribution of moss,132 cases(60.3%)were white,84 cases(38.4%)were yellow.In the distribution of moss,106 cases(48.4%)were thin,47 cases(21.5%)were greasy,and 42 cases(19.2%)were thick.(5)The Relationship of Body Composition and Tongue Image in MAFLD Patients with T2DM:Based on the color of the tongue,Qi deficiency constitution is mainly pale red tongue and pale white tongue,and there is a strong correlation between wet heat constitution and yin deficiency constitution.The color of the tongue was lilac,purple,and ecchymosis.On the shape of the tongue,Qi deficiency,phlegm,dampness and heat are the main factors of fat tongue and tooth mark,blood-stasis tongue is more moderate,yin deficiency is strongly related to thintongue and crack tongue.In terms of fur color,Qi deficient quality is mainly white,wet heat,yin deficiency and yellow hair are related.In terms of the properties of the fur,the main characteristics of Qi Deficiency and Blood Stasis are thin,damp,wet,thick and oily hair are related,but lack of hair is the main feature of Yin deficiency.Conclusion:(1)T2DM and MAFLD were significantly different in age and BMI.(2)The main constitution types of MAFLD patients with T2DM were Qi Deficiency,Phlegm,Dampness,Dampness and Heat.The major types of constitution were related to gender and BMI.(3)The main tongue color of T2DM patients with MAFLD were light red tongue and red tongue,the main tongue shape were tooth-mark tongue and fat tongue,the main fur color were white fur and yellow fur,the main fur quality were thin fur,greasy fur and thick fur.(4)The main constitution of MAFLD patients with T2DM was related to the tongue image. |
