Objective: The purpose of this study is to compare the clinical effects of staged rehabilitation exercises under the guidance of "tendon and bone" theory and rehabilitation exercises under the guidance of the "painless" principle in patients with intercondylar femoral fractures,in order to provide effective medical solutions and references for clinical rehabilitation treatment. The results of the study will be evaluated by comparing the clinical data of the two groups of patients to assess their clinical efficacy and differences. Methods: From October 2020 to December 2022,we collected 60 patients with intercondylar femoral fractures in Shandong Province Chinese Hospital, all of whom met the inclusion criteria, and divided them into treatment and control groups of 30 cases each, with the grouping based on the random number table method.Prior to surgery, patients in both groups received decongestive and anticoagulant therapy, kept the affected limb braked, stopped taking medications the day before surgery, completed all preoperative examinations, and excluded contraindications to surgery.A parapatellar approach was used in both groups, and the LISS plate was selected as the internal fixation. In the postoperative rehabilitation process, patients in the treatment group were instructed to perform active-passive recovery training in phases using the theory of "tendon and bone together".The principle of "the painless" guides the patient to perform regular active rehabilitation training.Three days after the stitches were removed, patients in both groups could receive herbal fumigation as an auxiliary treatment to invigorate blood circulation and relieve pain, and the formula was chosen to invigorate blood circulation and relieve pain.Statistical analysis of the perioperative data was performed for the two groups of patients in four time periods: 1 month,3months,6 months and 12 months after surgery. The Hospital for Special Surgery knee score(HSS),visual analogue score(VAS),Kolmert function, excellent rate, and complication rate were used to compare the clinical efficacy of the two rehabilitation exercises with reference to the data analysis results.Results:1.In terms of preoperative general information and perioperative-related information, there was no significant difference between the two groups of patients, and the difference was not statistically significant(P>0.05).2.After comparing the HSS scores, both groups showed significant improvement, and the difference was statistically significant(P<0.05);the HSS scores obtained in the treatment group were significantly higher than those in the control group at different periods of 1 month,3 months,6 months and 12 months after surgery, and the difference was statistically significant(P<0.05).3.After the comparison of VAS scores, the scores of both groups decreased significantly with time, and the difference was statistically significant(P<0.05),the scores of the treatment group were significantly lower than those of the control group at the first and third months after surgery(P<0.05),there was no significant difference between the scores of the two groups at the sixth and twelfth months after surgery(P>0.05).4.In the final follow-up,the HSS score showed that the excellent rate in the treatment group(86.67%)was significantly better than that in the control group(60%),and the difference was statistically significant(P<0.05);the Kolmert functional assessment results also showed that the excellent rate in the treatment group(86.67%)was significantly better than that in the control group(63.33%),and this difference was statistically significant(P<0.05));after statistical data analysis, postoperative complications occurred in both groups, but the incidence rate in the treatment group was clearly lower than that in the control group,reaching(13.33%),while the incidence rate in the control group was(36.67%),and the difference was statistically significant(P<0.05).Conclusion: 1.The postoperative rehabilitation exercises guided by the theory of "tendon and bone" can reduce the incidence of postoperative complications and effectively promote the recovery of functional activities of the knee joint, and the fumigation treatment with traditional Chinese medicine can achieve significant clinical efficacy, which can provide new options and new ideas for the postoperative functional rehabilitation of such fractures.
