Objective: To carry out the popularization and application research on the diagnosis and treatment scheme of combined use of traditional Chinese medicine and Western medicine for children with community-acquired pneumonia in acute stage in primary hospitals,and to evaluate the clinical applicability of the scheme.Methods:From November 2021 to October 2022,hospitalized children who met the diagnostic criteria for acute community-acquired pneumonia in five prefecture-level general hospitals in the province were selected as the research objects.Treatment based on syndrome differentiation was performed according to the diagnosis and treatment scheme,and the score changes of Children’s Pneumonia Scale(see Appendix 1)were recorded and statistically analyzed during the medication period,so as to objectively evaluate the practical application of this scheme in primary hospitals.Results:1.Among the children,56.23% were male and 43.77% were female,and the children were mainly between 3 and 9 years old.2.12.85% of the patients had a history of recurrent pneumonia,allergic rhinitis,recurrent upper respiratory tract infections,and 82.64% of the patients had a history of 1-2times of pre-treatment,including antibiotics,antitussive and expectorant drugs,hormones and so on.3.Among the main symptoms of pneumonia,pulmonary rales(97.22%)accounted for the most,of which moderate and fine moist rales accounted for 85.76%,followed by expectoration(96.18%),obvious cough(95.14%),fever(89.24%)and obvious wheezing(32.64%).Pneumonia-related symptoms such as anorexia(58.67%),nasal obstruction and runny nose(36.81%),poor sleep(36.11%)were more common.The main clinical signs were pharyngeal congestion(97.91%),abnormal complexion(28.47%)and tonsillar enlargement(15.28%).4.Mycoplasma pneumoniae,influenza a virus and influenza B virus infection accounted for 76.17%,6.54% and 4.67% of the total detected children,respectively,and the detection rate of Mycoplasma pneumoniae in the 6-14 age group was higher than that in other age groups.Mixed infection accounted for 8.41%.5.During the period of enrollment,antibiotics were used frequently in western medicine,and azithromycin,erythromycin lactobionate and cephalosporins were the main drugs.The Chinese patent medicines and traditional Chinese medicine preparations used mainly include Xiyanping,Reduning and Tanreqing.6.There was a significant difference in the scores of main symptoms of pneumonia before and after treatment(P < 0.05).In this study,the cure rate was 25.69%,the effective rate was 49.31%,the effective rate was 24.31%,and the ineffective rate was 1.04%.The overall effective rate of the scheme was 98.96%.There were significant differences among the three groups(P < 0.05),and the descending score of the wind-heat blocking lung group was better than that of the damp-heat blocking lung group.7.There was a significant difference in TCM syndrome scores before and after treatment(P < 0.05).The incidence of adverse drug reactions was low,and there were no serious adverse reactions.The incidence of severe disease was low at the end of the trial.8.After treatment,the median and quartile of time to complete resolution of fever was24(10,48)hours,the mean and standard deviation of time to onset of cough was 3.21 ±0.95 days,the mean and standard deviation of time to onset of expectoration was 3.52 ±0.95 days,and the median and quartile of time for onset of asthma was 2(1,2)days.Abnormally elevated white blood cell count,neutrophil count,neutrophil percentage,CRP and other inflammatory indicators were reduced.Conclusion:1.This program has good antipyretic,antitussive,expectorant and antiasthmatic effects.After medication,the main symptoms of pneumonia such as fever,cough,expectoration and wheezing were alleviated,and the corresponding secondary symptoms and TCM syndromes were also alleviated to varying degrees.2.The use of integrated traditional Chinese and western medicine in the treatment of children with community-acquired pneumonia in acute stage can significantly improve the symptoms and signs of children with pneumonia,the overall effective rate is high,and the test drug is safe and reliable.
