| Objective To observe the effect of Xing Pi Changwei Prescription on postoperative quality of life after chemotherapy in high-risk stage Ⅱand Ⅲ colorectal cancer patients.Methods Thirty-two high-risk patients with stage Ⅱ and Ⅲcolorectal cancer who met the inclusion criteria and were admitted to the Oncology Department of Yantai Hospital of Traditional Chinese Medicine from July 2021 to November 2022 after radical resection were enrolled in this study.A randomized self-cross-control study was adopted and 32 patients were divided into AB group and BA group by random number table method.Two cycles of chemotherapy were continuously observed.Both groups were treated with XELOX chemotherapy.Group A was treated with chemotherapy combined with traditional Chinese medicine Xingpi Changwei prescription;group B was treated with chemotherapy combined with placebo.Digestive tract reaction score(NCI digestive tract reaction scale),TCM syndrome score(abdominal distension and epigastric distress,numbness,nausea,loose stool or diarrhea,heavy body and limbs),MDASI scale,KPS score and fecal flora distribution of patients in 2 groups before and after chemotherapy were observed.Results There was no statistical difference in baseline level between group AB and group BA,which was comparable.1.Digestive tract reaction:After chemotherapy,nausea and vomiting in the treatment group were significantly different from those in the control group(P<0.01),the diarrhea in the treatment group was significantly improved compared with the control group(P<0.05).2.TCM syndrome: After chemotherapy,intra-group comparison: total score of treatment group and control group was significantly reduced(P<0.01).Comparison between groups: The total score of treatment group and control group was significantly different(P<0.01).3.KPS score: After chemotherapy,the KPS score of the treatment group and the control group had a very significant difference,and the curative effect improvement had a very significant difference(P<0.01).4.MDASI scale: After chemotherapy,fatigue and diarrhea in the treatment group were significantly improved compared with the control group(P<0.05),and nausea,appetite,vomiting and general activity were significantly improved(P<0.01).5.Fecal flora: Through 16 S high-throughput sequencing analysis of fecal flora samples,a total of671 OTUs were generated.Compared with the control group,fecal flora richness of patients after chemotherapy was significantly increased(P<0.05)with statistical difference.At the phylum classification level,the dominant species of fecal flora samples in the two groups did not change much before and after chemotherapy,but at the genus classification level,the top four dominant species in fecal flora samples in the two groups were different before and after chemotherapy.At the phylum classification level,the proportion of Firmicutes and Bacteroidetes increased in the treatment group,while the proportion of actinomyces and Proteobacteria decreased,and the overall structure of the control group did not change significantly.At the level of genus classification,the proportion of Escherichia coli decreased and the richness of Lactobacillus increased significantly in the treatment group.Xingpi Changwei can help patients recover intestinal flora disorder,improve the richness of beneficial bacteria Lactobacillus and reduce the richness ratio of conditioned pathogenic bacteria Proteobacteria.Conclusion The combination of Xing Pi Changwei Prescription with chemotherapy can significantly improve the Gastrointestinal Response Score,Chinese Medicine Symptom Score,KPS Score and MDASI Scale Score of high-risk stage II and III colorectal cancer patients with "spleen deficiency and phlegm obstruction",reduce the adverse effects of chemotherapy such as nausea,vomiting,nausea and diarrhea,restore the disorder of intestinal flora caused by surgery and chemotherapy,and increase the proportion of beneficial bacteria and reduce the proportion of pathogenic bacteria in the fecal flora of high-risk stage II and III colorectal cancer patients. |
