| Objective : To explore the sleep parameters related to polysomnography(PSG)of insomnia with liver-fire disturbing heart syndrome and heart-gallbladder qi deficiency syndrome in traditional Chinese medicine.Through the difference and regularity of PSG sleep parameters between the two syndromes,the quantitative criteria of syndromes were found to further guide clinical more accurate syndrome differentiation and prescription selection.Methods : According to the inclusion and exclusion criteria,a total of 60 patients were screened,including 30 patients with liver-fire disturbing heart syndrome and 30 patients with heart-gallbladder qi deficiency syndrome.The general data and Pittsburgh Sleep Quality Index(PSQI)scores of the two groups were recorded,and the polysomnography(PSG)sleep-related parameters of the two groups were observed and compared.Including sleep process parameters(including total sleep time(TST),sleep latency(SL),sleep efficiency(SE),number of awakenings(AT))and sleep structure parameters(including non-rapid eye movement(NREM)sleep each period(N1,N2,N3)time and percentage of total sleep time(N1 %,N2 %,N3 %),rapid eye movement(REM)sleep time(R)and percentage of total sleep time(R %)),a total of 12 items,and statistical methods were used to analyze and compare the characteristics and differences of polysomnography between the two groups of liver fire disturbing heart syndrome and heart and gallbladder qi deficiency syndrome.Results : Combined with the exclusion criteria,59 cases were finally obtained,30 cases in the liver fire disturbing heart group and 29 cases in the heart and gallbladder qi deficiency group(1 case was lost).1.General information : Age,gender and BMI were statistically tested,and there was no significant difference between the groups(P > 0.05).Finally,59 patients(21 males and 38 females)were included,30 patients(8 males and 22 females)in the liver fire disturbing heart group,and 29 patients(13 males and 16 females)in the heart and gallbladder qi deficiency group.Among them,women accounted for a relatively high proportion,especially in the liver fire disturbing heart group.2.There was no significant difference in subjective sleep between the two groups in the past month(P > 0.05).3.PSG sleep data :a total of 12 sleep parameters.(1)Comparison of sleep process : The TST and SE of the heart and gallbladder qi deficiency group were higher than those of the liver fire disturbing heart group.The difference between the two groups in TST was statistically significant(P < 0.01),and the difference in SE was statistically significant(P < 0.05).There was no significant difference in SL and AT(P > 0.05).SE of the two groups was significantly lower than the normal value,AT was significantly higher than the normal value,and sleep latency was roughly normal.In addition,TST of the liver fire disturbing heart group was significantly lower than the normal value;(2)Comparison of sleep structure : The N2 and N2 % of the heart-gallbladder qi deficiency group were higher than those of the liver-fire disturbing heart group,and the N3 and N3 % were lower than those of the liver-fire disturbing heart group.The difference in N2 % was statistically significant(P < 0.05).The difference in N2,N3 and N3 % was statistically significant(P < 0.01).There was no significant difference in N1,R,N1 % and R %(P > 0.05).Both of them have the disorder of NREM sleep structure,which is manifested as an increase in shallow sleep period(N1 + N2 period)and a decrease in deep sleep period(N3 period).Conclusion : 1.There was no statistical difference in PSQI between the two groups,and the two syndromes could not be identified by subjective sleep.2.There are differences in PSG sleep parameters between insomnia patients with liver-fire disturbing heart syndrome and heart-gallbladder qi deficiency syndrome,which are manifested in TST、SE、N2、N2%、N3、N3%. |
