| Objective: To study the value of hyperbaric oxygen in the treatment of severe pneumonia complicated with sepsis in children.Methods: Select children with severe pneumonia combined with sepsis in Dalian Women and Children’s Medical Center Group from November 2021 to December2022,and randomly divided into 2 groups according to the numerical table method: 30 cases of control group(conventional therapy group),and 30 cases of treatment group(conventional therapy + hyperbaric oxygen group).Patients in the control group were given conventional treatment,including anti-infection,Small dose of heparin anti-inflammatory anticoagulation,oxygen inhalation,sputum conversion,nutritional support,and maintenance of water,electrolyte and acid-base balance.The observation group received hyperbaric oxygen therapy on the basis of the control group treatment,1session / day,7 days for 1 treatment course.Dynamic assessment of days 1,3,and 7 of WBC,CRP,PCT in the treatment and control groups.The IL-10,IL-27,D2 polymers,FDP and other indicators,compare and analyze the differences between the treatment group and the control group.Results:1.General data,including sex,age,primary WBC,CRP,PCT,chest X-ray and oxygen inhalation,blood oxygen saturation,pathogen culture positive rate,the differences were not statistically significant(P> 0.05).2.The comparison of WBC,CRP and PCT between the two groups showed that on the third day of treatment,WBC and PCT indexes of the treatment group were decreased significantly,and were statistical significantly lower than those of the control group(P<0.05).On the 7th day of treatment,WBC,CRP and PCT of the treatment group were significantly lower than those of the control group,with statistical significance(P<0.05).3.The comparison of the coagulation index D-dimer and FDP between the two groups showed that the D-dimer and FDP in the treatment group were significantly lower than that of the control group(P<0.05)4.The comparison of the increase or decrease ratio of various indexes between the two groups on the third day of treatment showed that the IL-27 index of the treatment group was significantly decreased compared with the control group,and IL-10 was significantly increased,with statistical significance(P<0.05).The comparison of the increase or decrease ratio of various indexes between the two groups on the 7th day of treatment showed that IL-27 in the treatment group continued to decrease and IL-10 continued to increase compared with the control group,with statistical significance(P<0.05).5.The prognosis of the children in the two groups,including antipyretic time,oxygen absorption time,chest film absorption time and hospitalization days,showed that the antipyretic time,oxygen absorption time,absorption time and hospitalization days of chest film were better than that of the control group,which was statistically significant(P<0.05).6.The correlation coefficient analysis of changes and hospital stay,antipyretic time,oxygen inhalation time and chest X-ray absorption time on day 3 and day 7 of treatment groups showed that the proportion of IL-10 and hospital stay and absorption time after hyperbaric oxygen treatment were significantly negatively correlated(correlation coefficient <0),and the degree of IL-27 decline was positively correlated with the four evaluation indexes(correlation coefficient> 0).Conclusion:1.Hyperbaric oxygen therapy can early inhibit the growth of inflammatory markers.IL-10 and IL-27 can be used as indicators for early identification of serious infectious diseases.2.Hyperbaric oxygen therapy can improve the coagulation function of children with severe pneumonia and sepsis early.3.Hyperbaric oxygen therapy can improve hypoxemia in patients with severe pneumonia with sepsis,promote the absorption of lung inflammation,and ultimately reduce the length of hospital stay in patients with severe pneumonia with sepsis. |
