| Objective:1.To discover the clinical efficacy and safety of lysine zinc gluconate granules combined withconventional therapy in thetreatment of acne vulgaris.2.To detect the expression of TNF-a and IL-6 in acne vulgaris patients and analyze the correlation between serum levels of them andthe severity of acne.Method:Patients diagnosed as acne vulgaris in department of Dermatology of Northern Jiangsu People’s Hospital from October 2021 to July 2022 were collected and divided into two categories according to theacne Pilsbury four-class classification,in which mild class I and moderateclass II were category I,moderate class III and severe class IV were category II.The conventional topical treatment for category I patients was fusidic acid cream and Adapalene Gel only,in which category II patients were treated with oral Doxycycline Hyclate Capsules in addition to topical fusidic acid cream and Adapalene Gel.80 subjects were randomly divided into two groups,the observation group and the control group,with 40 cases in each group.The observation group received oral lysine zinc gluconate granules combined with conventional topical treatment,while the control group was treated with conventional topical treatment only.The patients were evaluated for curative effect and quality of life improvement before and after six weeks treatment,and the recurrence rate and the occurrence of adverse reactions were recorded.Meanwhile,serum of 40 patients with acne vulgaris attending the outpatient clinic of the Dermatology Department of Northern Jiangsu People’s Hospital were collected designed as the acne group,and another serum of 20 cases from health check-up centers were designed as the control group.The serum levels of IL-6 and TNF-a of all subjects were tested by enzyme-linked immunosorbent assay(ELISA),and theresults werecompared and analyzed.Results:1.Comparison of clinical efficacy of lysine zinc gluconate granules combined with conventional therapy in the treatment of acne vulgaris:After treatment,the total number of lesions,the number of inflammatory lesions and the number of non-inflammatory lesions were reduced in both groups;thetotal effective rate of the observation group was 95%,among which 2 cases were cured,18 cases showed efficacy,18 cases were improved and 2 cases were ineffective;The overall effeciency of the control group was 70%,among which 1 case was cured,3 cases showed efficacy,24 cases were improved and 12 cases were ineffective.The clinical efficacy of the observation group was better than that of the control group,(P<0.05).Acne severity [GAGS score] and Dermatology Quality of Life Index [DLQI score] scores in both group were decreased compared with those before treatment,but the decrease was greater in the observation group(P<0.05).2.Recurrence and safety comparison: All participating patients completed follow-up.The recurrence rate of the observation group(13.16%)was lower than that of the control group(39.29%),and the difference was statistically significant(P<0.05),the incidence of adverse reactions was similarinbothgroups,and no serious adverse reactions occurred.3.Analysis of serum TNF-a and IL-6 levels in patients with acne vulgaris and their correlation with the severity of the disease: 40 patients with acne vulgaris,including 12 cases of mild,16 cases of moderate and 12 cases of severe acne,The serum IL-6 levels of all patients in acne group were higher than that of the control group(P<0.05),while the difference in IL-6 levels between the groups of patients with different severity was not statistically significant.As for the TNF-a levels of the patients,the results in the moderate and severe groups were higher than that in the control group,and the same is true between the severe group and mild group(P<0.05).However,there were no significant differences in serum TNF-a levels between the mild and control groups,the mild and moderate groups,and the moderate and severe groups(P>0.05).Conclusion:1.Lysine zinc gluconate granules combined with conventional therapy can effectively improve the skin lesions and improve the quality of life of patients with acne vulgaris,and the recurrence rate after medication is lower thanthat of the control group with high safety.2.IL-6 was involved in the inflammatory response of acne,but it was not related to the severity of acne and could not be a marker used to assess the severity of acne;TNF-a was involved in the cascade response of acne inflammation and its level was positively correlated with theseverity of acne. |
