Clinical Study On Analgesia After Caesarean Section By Electroacupuncture Combined With Ear-Point Pressure Bean

Objective:To investigate the analgesic effect of electroacupuncture combined with auricular point pressing bean assisted patient controlled intravenous analgesia(PCIA)after cesarean section,and its effect on postoperative lactation,first anal exhaust and postoperative adverse reactions,so as to provide a new idea for clinical research on analgesia after cesarean section.Methods:According to the principle of randomized control,60 parturients who met the inclusion criteria were divided into observation group(30 cases)and control group(30 cases)by random number table method.In the control group,after cesarean section,a one-time intravenous self-controlled analgesia pump was connected.The drug in the pump was diluted to 100 m L with sufentanil 2μg/kg and 0.9% sodium chloride solution.The parameters were set as follows : load 3ml/h,maintenance dose 2m L/h,self-controlled dose 0.5m L,time locking 15 min,and uniform input until pain relief.If the maternal pain is severe,the button of the patient-controlled intravenous analgesia pump(temporary regulation)can be pressed on its own,and removed after 48 hours.Observation group : On the basis of the treatment of the control group,electroacupuncture combined with auricular point pressing was adopted.Electroacupuncture treatment : 12 acupoints of Hegu,Neiguan,Zusanli,Diji,Sanyinjiao and Taichong on both sides were selected for electrical stimulation.Hegu and Neiguan on the same side were connected to one group,Zusanli and Diji on the same side were connected to one group,Sanyinjiao and Taichong on the same side were connected to one group,with a frequency of 2/100 HZ.The waveform was selected as the density wave,and the current intensity was based on the tolerance of the parturient.The stimulation was performed for 30 min,and the operation was performed at 2h,12 h,24h,36 h and 48 h after operation.Auricular pressure bean treatment : select unilateral uterus,pelvic cavity,sympathetic,Shenmen,subcortical,with the index finger and thumb pulp relative pressing,pressing when there is a sense of soreness and pain,the degree of tolerance,each pressing 1-2min,2h after the operation,maternal pain increased,can be self-kneaded by themselves or their families,retained for 48 h.The degree of pain,lactation,first anal exhaust,Number of analgesic pump compressions,life signs and adverse reactions were observed at 2h,12 h,24h,36 h and 48 h after operation,and the clinical efficacy and safety were evaluated.The clinical observation data were analyzed by SPSS26.0.Results:1.Baseline data : In this experiment,the age,BMI,gestational age and operation time of the two groups before treatment were compared,and the difference was not statistically significant(p > 0.05).2.Comparison of postoperative pain scores : Comparison within the group,the control group and the observation group were compared with the Visual Analogue Scale(VAS)scores at 2h and 12 h,12h and 24 h,24h and36 h,36h and 48 h after surgery,and the difference was statistically significant(p < 0.05).There were significant differences in pain scores between the two groups at 2h,12 h,24h,36 h and 48 h after operation(p < 0.05).The pain score of the observation group was significantly lower than that of the control group.3.Comparison of lactation score : within the group,compared with the lactation score at 24 h after operation,the lactation score at 48 h after operation of the two groups increased,and the difference was statistically significant(p< 0.05);There was no significant difference in lactation score between the two groups at 24 hours after operation(p > 0.05).the difference of lactation score at 48 h after operation was statistically significant(p < 0.05),and the lactation score of the observation group was higher than that of the control group.4.Comparison of the first anal exhaust time : By comparison,the first anal exhaust time of the observation group was shorter than that of the control group,and the difference was statistically significant(p < 0.05).5.Comparison of pressing times of analgesic pump : By comparison,the pressing times of analgesic pump in the observation group were less than those in the control group,and the difference was statistically significant(p <0.05).6.Comparison of postoperative vital signs : After comparison,the oxygen,heart rate and mean arterial pressure in the observation group and the control group were stable,with no statistical significance(p > 0.05).7.Comparison of postoperative adverse reactions: nausea and vomiting occurred in 5 cases in the control group and 1 case in the observation group within 48 h after obstetric surgery,and urine retention occurred in 1 case in the control group.No itching symptoms occurred in both groups.After comparison,there was statistical significance in the incidence of adverse reactions between the two groups(p < 0.05).8.Comparison of clinical efficacy: After comparison,the excellent and good rate of postoperative analgesia in the observation group was 96.67%,which was better than 83.33% in the control group,and the difference was statistically significant(p < 0.05).Conclusion:1.Electroacupuncture combined with auricular point pressing beans assisted patient-controlled intravenous analgesia can reduce the degree of pain after cesarean section,reduce the number of pressing times of patient-controlled intravenous analgesia pump,and have better analgesic effect;2.Electroacupuncture combined with auricular point pressing beans assisted intravenous controlled analgesia can increase the amount of lactation after cesarean section,help maternal anal exhaust as soon as possible,and reduce adverse reactions after cesarean section.3.Electroacupuncture combined with auricular point pressing beans assisted intravenous patient-controlled analgesia for pain after cesarean section has high excellent and good rate,rapid onset,safety and reliability.
