Development Of The Chinese Version Of The Oxford Arthroplasty Early Change Score (OACS) And The Preliminary Application

Abstract/Summary:
Objective1.To introduce and translate Oxford Arthroplasty Early Change Score(OACS)into Chinese and cross-cultural adaptation to form the Chinese version of OACS,and to evaluate its applicability,reliability and validity.2.This study aimed to conduct a cross-sectional investigation using the Chinese version of OACS to preliminarily understand the postoperative early recovery status of patients undergoing joint replacement surgery.Methods1.After obtaining permission from the original authors and institutions,the English version of OACS was translated,harmonized,back-translated,expert-reviewed,teamreviewed,pilot-tested and subjected to cross-cultural adaptation according to the guidelines provided by Oxford University Innovation to form the Chinese version of OACS.Using convenience smpling,a total of 168 orthopedic patients who had received joint replacement surgery were selected as study subjects from two Grade Ⅲ-A hospitals in Suzhou for investigation.The survey instruments included a general information questionnaire,the Chinese version of the OACS,and the Quality of Recovery-15.Data were analyzed using SPSS 26.0 software.Item discrimination was examined using the cutoff method,and item homogeneity was analyzed using Pearson correlation coefficient.Content validity,structural validity,and criterion-related validity were assessed using content validity index,exploratory factor analysis,and Spearman correlation analysis,respectively.Reliability was evaluated using Cronbach’s alpha coefficient and split-half reliability.2.Convenience sampling was used to select 150 orthopedic patients who had received joint replacement surgery from two Grade Ⅲ-A hospitals in Suzhou as study subjects.A general information questionnaire and the Chinese version of the OACS were used to investigate the recovery status of patients on the fourth day after surgery,and to preliminarily analyze the factors influencing early postoperative recovery.Results1.The translation guidance manual recommended by the scale development organization was followed to complete the translation work and produce the Chinese version of the OACS.The reliability and validity test results of the Chinese version of the OACS are as follows:item analysis shows that the correlation coefficients between each item and the total score of the Chinese version of the OACS are between 0.449 and 0.825,and the results of the cutoff method show that the differences are statistically significant(P<0.001),and the critical ratio are all greater than 3.The validity analysis shows that the I-CVI ranges from 0.86 to 1,the S-CVI is 0.86.Exploratory factor analysis extractes three factors with a cumulative variance contribution rate of 71.224%.Criterion-related validity analysis indicates that the correlation coefficient between OACS and the Quality of Recovery-15 is 0.423.Reliability analysis shows that the Cronbach’s α coefficient of the total scale is 0.923,the Cronbach’s α coefficients of each dimension are between 0.838 and 0.915.The split-half reliability is 0.925.2.A total of 155 questionnaires were distributed and retrieved in this study,with five invalid questionnaires removed and a valid response rate of 96.77%.Of the respondents,42(28%)were male,and 108(72%)were female,with an average age of 67.00±10.99 years.The total score of early postoperative recovery quality in joint replacement patients was1.93±21.27,with an average score of 1.63±7.26 in pain dimension,-3.10±7.66 in physical function dimension,and-0.46±8.89 in psychological and social status dimension.Among the items of the scale,item 7("Your pain in the affected area compared to before the operation?")had the highest score of 0.39±1.20,while item 3("Your ability to walk upstairs compared to before the operation?")had the lowest score of-0.83±1.06.Results of the univariate analysis indicated that there were significant differences among patients with different numbers of children in terms of the total score and psychological and social status dimension score of OACS(P<0.05),while patients with different surgical methods had significant differences in pain dimension score and physical function dimension score(P<0.05).Conclusion1.This study successfully developed a reliable and valid Chinese version of the Oxford Arthroplasty Early Recovery Score(OACS)questionnaire by translating and validating the English version.The Chinese version can be used to assess physical function and changes in physical symptom sensation in patients undergoing joint replacement surgery in China.It will provide medical professionals with a useful tool for evaluating patient early postoperative recovery quality and facilitate the promotion of speedy postoperative recovery.2.The quality of early postoperative recovery of patients undergoing joint replacement surgery is not high,and the number of children and surgical methods are factors affecting early postoperative recovery.Medical professionals should pay attention to patients with no children or only one child and take into account the differences in early postoperative recovery between patients with different operation types.Personalized intervention measures should be implemented to improve patient postoperative recovery and promote rapid recovery.
