| Objectives To explore the correlation between FDP,D-dimer,TNF-αand cTnI and aortic dissection on the basis of CTA,and further analyze the application value of laboratory test indicators of FDP,D-dimer,TNF-αand cTnI in evaluating the prognosis of patients with aortic dissection,providing a favorable reference for clinicians in the diagnosis and treatment of aortic dissection.Methods A total of 281 patients with"acute chest pain"admitted to The Affiliated Hospital of North China University of Science and Technology from January 2015 to January 2020 were selected.CTA examination and FDP,D-dimer,TNF-αand cTnI laboratory tests were carried out.According to the diagnostic results of CTA study divided into group of aortic dissecting aneurysm and aortic dissection group,according to the prognosis of patients with sandwich in the aortic dissection group outcome will be further divided into survival group and death group,respectively,the general data of two groups of research object and the FDP,D-dimer,TNF-αand cTnI laboratory test index statistics analysis,The ratio represents the data of counting,which is analyzed byχ~2test.For the data subject to the normal distribution analyzed by Kolmogorov-Smirnov test,is represented byx±s.For the data subject to the normal distribution,the independent sample t test is used for statistical analysis.For the data not subject to the normal distribution,is represented by the median quartile spacing[M(QR)].Mann-whitney U test was used for statistical analysis.taking advantage of the vascular imaging technology for integrated analysis of the aortic dissection,diagnosing the aortic dissection of parting and tore his mouth diameter measuring and observing the FDP,D-dimer,TNF-αand cTnI laboratory test index of expression aortic dissection was statistically significant with gender and history of hypertension.By drawing ROC curves,the cut-off values of FDP,D-dimer,TNF-αand cTnI in surviving and dead aortic dissection patients were analyzed,and the sensitivity and specificity were calculated.Results 1 Results of laboratory parameters in the aortic dissection group and the non-aortic dissection group:A total of 281 patients,aged 45 to 69 years old,with an average age of 57.53±5.10 years old,were divided into the aortic dissection group and the non-aortic dissection group according to the diagnostic results of CTA.The comparison of laboratory expression levels of FDP,D-dimer,TNF-αand cTnI between the two groups was conducted.The difference was statistically significant(Z=-13.453,-13.996,-13.995,-8.863,P<0.05).2 Results of laboratory parameters in patients with aortic dissection:A total of 104 patients,aged 45 to 69 years,with an average age of 57.48±5.11 years,were diagnosed with aortic dissection.According to the prognostic outcome of patients with aortic dissection,they were further divided into the death group and the survival group.The comparison of FDP,D-dimer,TNF-αand cTnI levels between the two groups was conducted.The difference was statistically significant(t=-7.429,-8.995,Z=-5.567,-5.768,P<0.05).3 Analysis of diagnostic characteristics of aortic dissection after CTA examination:the expression levels of FDP,D-dimer,TNF-αand cTnI were correlated with the maximum diameter of aortic dissection laceration,r were 0.390,0.534,0.818 and 0.466,respectively,and the differences were statistically significant(P<0.05),as indicated by scatter plot.There were significant differences in aortic dissection classification and maximum laceration diameter between the survival group and the death group(χ~2=25.713,Z=-6.433,P<0.05).There was no significant difference between the dissection type and gender and hypertension history(χ~2=5.077,3.540,P>0.05).4 The sensitivity and specificity of FDP were 91.3%and 82.7%,respectively.The sensitivity and specificity of D-dimer were 73.9%and 100.0%,respectively.The sensitivity and specificity of TNF-αwere 91.3%and 85.2%,respectively.The sensitivity and specificity of cTnI were 91.3%and 81.5%,respectively.5 The combined effect of FDP,D-dimer and TNF-αon the prognosis of patients with aortic dissection:the sensitivity and specificity of D-dimer combined with FDP were 82.6%and 87.7%,respectively.The sensitivity and specificity of D-dimer combined with TNF-αand FDP combined with TNF-αwere 95.7%and 90.1%,respectively.The sensitivity and specificity of FDP combined with TNF-αwere 95.7%and91.4%,respectively.Conclusions 1 Based on the basic study of CTA,it was found that the expression levels of FDP,D-dimer,TNF-αand cTnI were correlated with aortic dissection,which has certain application value in the diagnosis and treatment of aortic dissection.2 FDP,D-dimer,TNF-αand cTnI have high diagnostic efficacy in predicting the risk of hospital death in patients with aortic dissection,which can give better play to the advantages of laboratory detection
