Objective: The telocytes are a new type of interstitial cells involved in the regulation of smooth muscle contraction and tissue fibrosis.This paper studies the role of telocytes in the pathological changes of biliary dilatation.Methods: A total of 26 cases in the experimental group(biliary dilatation)and 6 cases in the control group who met the inclusion criteria were collected.MRCP was reviewed and classified according to the types of pancreaticobiliary maljunction.The corresponding bile duct was embedded in paraffin,sectioned and stained with HE to observe the pathological changes of bile duct.C-kit and tryptase immunofluorescence double staining were used to identify telocytes,and c-kit and caspase-3immunofluorescence double staining were used to detect telocytes apoptosis.Use Image-pro plus to analyze the distribution and quantity of telocytes and the expression of Caspase-3.The results were statistically analyzed by t?-test method.Results:1.In the experimental group,there were 23(88.5%)cases of pancreaticobiliary maljunction(11 cases of pancreaticduct type and 12 cases of bile duct type),and 3 cases of uncertain anatomical structure.2.In the experimental group,92.3% of the epithelium fell off in varying degrees;92.3% showed different degrees of inflammatory infiltration.The proportions of moderate and severe inflammation of bile duct type and pancreaticduct type were 66.7% and 36.7% respectively;65.5%showed no or scattered muscle tissue,and 34.5% showed discontinuous or continuous muscle tissue;All bile ducts showed different degrees of fibrosis.3.The telocytes are mainly distributed in the muscle layer of bile duct;the number of telocytes in the experimental group(14.6 ± 11.3)was lower than that in the normal group(50.6 ± 3.3),the difference was statistically significant(P < 0.05);the number of telocytes of bile duct type(9.5 ± 6.2)was lower than that of pancreaticduct type(19.5 ± 13.7),the difference was statistically significant(P < 0.05).The double positive cell percentages of C-kit and Caspase-3 in the experimental group and the control group were(8.9±8.6%,3.0±1.1%)respectively,and the difference was statistically significant(P <0.05).Conclusion: 1.In addition to bile duct dilatation,pancreaticobiliary maljunction is also an important anatomical feature of biliary dilatation.2.Epithelial exfoliation,inflammatory infiltration,muscle tissue reduction and fibrosis are important pathological changes of biliary dilatation.3.The telocytes are mainly distributed in the muscle layer of bile duct,which are reduced in the biliary dilatation,and the decrease of telocytes is more obvious in the bile duct type.The decrease of bile duct type is more obvious than that of pancreaticduct type,and the increase of apoptosis is one of the reasons for the decrease of telocytes.
