Aim:The retrospective analysis of the clinical data of hospitalized patients with Inflammatory Bowel Disease(IBD)with arthritis understood the relationship between disease activity and arthritis severity,and studied the correlation between C response protein(C reactive protein,C and IBD with arthritis disease activity and prognosis.Study methods:A total of 47 patients with inflammatory bowel disease complicated with arthritis admitted to the First Affiliated Hospital of Jinan University from 2016 to 2021 were retrospectively studied.The clinical information of the disease before and 6 months after treatment was summarized.atients with ulcerative colitis(UC)were evaluated by Mayo score,and patients with Crohn’s disease(CD)were evaluated by Crohn’s disease activity index(CDAI).Arthritis was assessed by Western ontario and mcmaster universities osteoarthritis index(WOMAC).To systematically evaluate the relationship between disease activity and severity of arthritis in patients with IBD.The C values before treatment and 6 months after treatment were collected for correlation analysis with the disease activity of UC and CD patients with arthritis.Statistical data were analyzed by S SS 22.0 statistical software.Results:1.Clinical data1.1 General clinical dataUC,CD two groups in age,gender difference was not statistically significant.In patients with UC and CD combined with arthritis,the discomfort symptoms were mainly digestive tract symptoms.In the relationship between intestinal symptoms and joint symptoms,intestinal discomfort symptoms were mainly prior to joint discomfort symptoms.The number of axial arthritis is more than peripheral arthritis.1.2 Laboratory test resultsThe comparative analysis between groups before and after the treatment of IBD patients with arthritis,the results showed that with the aggravation of IBD,white blood cell count(WBC)and neutrophil ratio(NEUT)showed an upward trend,while Albumin(ALB)showed a downward trend,with statistical significance(<0.05).After treatment,WBC and ALB in IBD mild,medium and heavy three groups were in the normal range and were not statistically significant(P>0.05).support Group comparative analysis of whether patients with IBD and arthritis had hematochezia,and the results showed that the red blood cell count(BC),Hemoglobin(HGB)were lower than the group without hematochezia(P<0.05).Differences between K~+and Na~+concentrations in different IBD groups,but not between Cl-concentrations,and K~+and Na~+loss in severe IBD patients.1.3 outine stoolWith the aggravation of IBD,the number of white blood cells in stool routine results gradually increased,and the number of people with normal results gradually decreased.1.4 EnteroscopyThe colonoscopy results of IBD patients showed that the severity grouping of intestinal lesions evaluated by IBD endoscopy before and after treatment was roughly consistent with the disease activity grouping of IBD score.1.5 Joint imagingThe results of joint imaging showed that the number of people with normal imaging results after treatment was higher than that before treatment,and the arthritis changes were alleviated except sacroiliac arthritis.2、The relationship between IBD disease activity and severity of arthritis:UC and CD groups were positively correlated with WOMAC score before treatment and after review,and the higher the disease score,the higher the arthritis index score(>0,P<0.05).4、The relationship between CRP and IBD activity,treatment efficiency:C and ΔC were positively correlated with the severity of clinical diseases before and after treatment(P<0.05,0<<1).C andΔC were correlated with the therapeutic effect of IBD,andΔC(1.156(95%CI),P=0.009)and C(1.015(95%CI),P=0.015)were risk factors for IBD.The area of OC curve drawn by C andΔC was 0.727 and 0.784,and the prediction accuracy of C andΔC on the therapeutic effect of IBD was medium.Conclusion:1.Interaction between IBD disease activity and severity of arthritis.2.C can be used as a predictor of IBD disease activity and therapeutic effect.
