Purpose:To compare the perioperative effect of total knee arthroplasty under computer navigation with traditional surgery in the treatment of knee osteoarthritis.Methods:To compare the perioperative effects of computer-assisted total knee arthroplasty(TKA)with traditional surgical methods in the treatment of knee osteoarthritis.This study was a retrospective analysis.According to the inclusion criteria,60 patients(30 males and 30 females)with knee osteoarthritis who needed unilateral total knee arthroplasty admitted to the Department of Joint Surgery of a Classⅲ Grade A hospital in Northeast China from June 2021 to September 2022 were selected.Among them,30 patients who underwent computer navigation knee arthroplasty were selected as the navigation group(male to female ratio 1:1),and 30 patients who underwent traditional knee arthroplasty were selected as the non-navigation group(male to female ratio 1: 1)Under the same perioperative comprehensive management measures,the perioperative clinical indications of the two groups were collected and recorded,including general vital signs,operation time,preoperative and postoperative blood routine counts,pain duration score,and knee flexion Angle.The total blood loss,intraoperative blood loss,hidden blood loss and Neutrophil-to-lymphocyte ratio(NLR)of each patient in the two groups were calculated,and the perioperative effects of the two groups under different surgical methods were compared and analyzed.Results:Among the enrolled cases,the average age of the navigation group was(67.57±6.23)years old,and there were 15 females(50%)and 15 males(50%).The average age of the non-navigation group was(64.90±5.49)years old,and there were 15 females(50%)and 15 males(50%).There was no significant difference between the two groups(p > 0.05).In navigation group,the average height was(164.86±7.36)cm,the average weight was(63.57±8.84)kg,and the average BMI was(24.21±4.48).In the non-navigation group,the average height was(166.93±8.20)cm,the average weight was(64.67±12.20)kg,and the average BMI was(23.16±3.81).There was no significant difference between the two groups(p > 0.05).The average operation time of the navigation group was 139.63±18.77 min,and that of the non-navigation group was 112.73±18.99 min.The difference between the two groups was statistically significant(P < 0.001).The hidden blood loss in the navigation group was(329.03±165.02)ml,and that in the non-navigation group was(468.6±198.52)ml.There was significant difference between the two groups(p=0.004),and the hidden blood loss in the navigation group was significantly lower than that in the non-navigation group.The intraoperative blood loss per unit time was(4.65±1.03)ml/min in the navigation group and(5.38±1.53)ml in the non-navigation group.The intraoperative blood loss per unit time in the navigation group was significantly less than that in the non-navigation group(p=0.034).The postoperative neutrophil-to-lymphocyte ratio(NLR)in the navigation group was7.28±3.19,which was significantly lower than that in the non-navigation group(10.96±7.42,p=0.015).Navigation with the navigation group,the total blood loss,intraoperative blood loss,postoperative 24 h average flow rate and postoperative leukocyte value,average preoperative postoperative neutral grain of value,the average postoperative HSS scoring value(at a for Special Surger Score,Hospital for Special Surgery pain score),preoperative and postoperative average visual analogue scale(VAS),the difference of thigh circumference at 1cm above the patella,and preoperative and postoperative knee flexion range of motion were not statistically different(all p>0.05).Conclusion:1.The average operation time of the navigation group was significantly longer than that of the non-navigation group,but from the perspective of the trend of change,with the increase of the number of surgical cases,the surgeons have passed the complete learning curve,and the average time has been close to that of the non-navigation group.(2)There was no significant difference in the total blood loss between the navigation group and the non-navigation group,but the hidden blood loss in the navigation group was significantly less than that in the non-navigation group,and the average blood loss in the navigation group was significantly less than that in the non-navigation group.In the future,when the operation time was not different,computer navigation may reduce the need for postoperative blood transfusion.(3)NLR in the navigation group was significantly lower than that in the non-navigation group.Under the condition of no difference in independent factors of infection,the risk of perioperative infection in the navigation group was lower than that in the non-navigation group.(4)The patients in the navigation group and the non-navigation group had the same pain experience,the same knee swelling changes,and the improved knee flexion range of motion.The perioperative effect of the two groups was the same,but the long-term evaluation still needs to be further improved.
