Objective:Compared with the traditional operation,to explore the clinical effect of Orthopilot computer assisted total knee arthroplasty.Method:The clinical data of 86 patients who underwent total knee arthroplasty in our hospital from December 2018 to June 2020 were analyzed retrospectively.The patients were divided into two groups:computer navigation group and traditional mechanical positioning group.43 patients(43 knees)in each group.The data of the two groups were compared.86 patients were treated with TKA for the first time.There were 11 male patients(11 knees)and 32 female patients(32 knees)in the navigation group,with an average age of(67.81±6.67)years.In the traditional group,there were10 male patients(10 knees)and 33 female patients(33 knees),with an average age of(68.07±7.04)years.The general data,such as gender,age,side of affected limb,type of arthritis,operative time,intraoperative blood loss and postoperative drainage at 72 hours were evaluated in the two groups.Postoperative imaging indicators,knee range of motion(ROM)score,New York Hospital for Special Surgery(HSS)functional score at 12 months and The pain visual analogue scale(VAS)scores of the patients were recorded 3 days after the operation,and statistical processing,analysis and comparison were performed.Results:Both groups had a mean follow-up time of 18 months,There was no significant difference in gender,age,affected limb side,arthritis type,preoperative joint range of motion,preoperative HSS score,preoperative VAS pain score,etc.(P>0.05);The operation time of navigation group(125.44±9.32)min was longer than that of traditional group(99.33±8.54)min,the difference was statistically significant(P<0.05);The amount of intraoperative bleeding in the navigation group(124.02±18.48)ml was less than that in the traditional group(177.02±21.94)ml,the difference was statistically significant(P<0.05);The postoperative drainage volume of the navigation group was(298.79±48.01)ml less than that of the traditional group(346.21±32.36)ml,the difference was statistically significant(P<0.05);The hip-knee-ankle angle(HKAA)deviation angle of the affected side in the navigation group after surgery 1(0.5-1.5)° was smaller than that in the traditional group by 3(2-3)°,and the postoperative coronal femoral component angle(CFCA)deviation angle of 1(0.5-1.5)° in the navigation group after surgery was smaller than that in the traditional group by 1.5(1-2.5)°,The deviation angle of 1(0.5-1.5)° of coronal tibial component angle(CTCA)in the navigation group was less than 2(1-2)°in the traditional group,and 1(1-2)° of sagittal femoral component angle(SFCA)in the navigation group was less than 2(1-2)° in the traditional group,The deviation angle 1(0.5-1.5)° of sagittal tibial component angle(STCA)in the navigation group was less than 2(1-2)°in the traditional group,and the deviation angle 1(1-2)° of posterior tibial slope(PTS)in the navigation group was less than 3.5(2.5-4)°in the traditional group(P<0.05).The ROM of the knee joint in the navigation group(107.88±4.64)° was better than that in the traditional group(100.09±4.73)° with significant difference(P<0.05);The postoperative HSS score of navigation group(90.51±1.44)was better than that of traditional group(87.21±1.57)(P<0.05);The postoperative VAS pain score of 4(3-4)in the navigation group was better than that of 5(4-5)in the traditional group(P<0.05).Conclusion:Compared with traditional TKA,TKA assisted by Orthopilot computer navigation is beneficial to reduce intraoperative blood loss and postoperative drainage,provide more accurate prosthesis placement and lower limb alignment,and obtain better knee ROM and postoperative HSS score.and postoperative VAS score,but prolonged operation time.
