| Objective: To observe the efficacy of colchicine in the treatment of skin lesions in children with IgA vasculitis(IgAV),and its effect on immunological indicators.Methods: A total of 71 children with IgAV who visited the Second Xiangya Hospital of Central South University from January to November2021 were selected.The subjects were divided into a therapy group and a control group according to whether they received colchicine nor not.The conventional treatment methods of the two groups were the same,and both groups were followed up for 6 months.The remission and the recurrence of skin lesions,as well as the incidence of adverse events of the two groups were observed.The clinical data of the children were collected,and the ratios of neutrophil/lymphocyte and monocyte/lymphocyte were calculated based on the routine blood tests.The proportion of lymphocyte subsets and Th1/Th2/Th17 cytokines in peripheral blood were detected before and after 1 month of treatment,and the immune indexes in different stages of the disease and between groups were compared and analyzed.Results:1.Baseline data comparison: There were no significant differences in basic information such as age,gender,disease course distribution,clinical phenotype,and co-infection between the two groups(P>0.05).2.Efficacy analysis:(1)There were no significant differences in the rash fading time,the rates of remission and recurrence between the two groups(P>0.05);(2)11 children with refractory skin lesions showed a significant decrease in the recurrences of rash after colchicine treatment(P<0.05);(3)The overall IgAV patients showed that the proportion of B cells in the remission stage was significantly lower than that in the acute stage(P<0.05),and there was no significant difference in the degree of decline between the two groups(P>0.05);(4)After treatment,the IL-2level in the therapy group was lower,and the IL-10 level was significantly higher than that in the control group(P<0.05).3.Safety analysis: There was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusions:Colchicine has no obvious efficacy in the treatment of acute skin lesions of IgAV,it may reduce the recurrence of refractory IgAV with good safety.Colchicine may play an anti-inflammatory effect in IgAV by affecting the expression of IL-2 and IL-10. |
