Objective: To investigate the relationship between the peripheral blood single nucleated cell exosome miR-155 and pro-inflammatory M1-type macrophage polarization in patients with lupus nephritis.Methods:(1)Peripheral blood of 39 people diagnosed with lupus nephritis clinically or by renal pathological examination in the Affiliated Hospital of Youjiang Medical College for Nationalities from April 2020 to December 2022 was collected.In addition,20 blood samples from healthy examiners were collected as a control group.(2)RT-PCR was used to determine the expression of miR-155 in exosomes of PBMCs.(3)The ELISA method was used to detect M1,M2 macrophage marker factors and inflammatory factors in the collected samples.(4)Collect the relavant clinical indexes of the collected sample patients.(5)The correlation between miR-155-5p and TNF-α,i NOS,Arg-1,miR-155-5p and IL-1β,IL-18,TNF-α,i NOS,Arg-1 and clinical indicators,IL-1βand IL-18 was analyzed.Results:(1)miR-155-5p was expressed in PBMCs exosomes,and miR-155-5p in PBMCs exosomes in patients with severe activity group had the highest expression,and the expression difference between the whole groups was statistically significant(P<0.05).(2)The expression of miR-155-5p was negatively correlated with the concentration of i NOS,an associated protein of M1-type macrophages(r=-0.427,P=0.033).(3)The concentrations of cytokines TNF-α and i NOS related to M1 macrophages,and were positively correlated with the concentrations of inflammatory factors IL-1β and IL-18.concentration of TNF-α was positively correlated with SLEDAI,but not with other clinical data.The concentration of i NOS was positively correlated with SLEDAI,anti-ds DNA concentration and ANA concentration,but not with complement C3 level.Conclusion:(1)Exosomal miR-155 was highly expressed in PBMCs from patients with lupus nephritis.(2)M1 macrophage polarization was increased in patients with lupus nephritis,and the degree of polarization correlated with the activity of the disease.(3)There was a correlation between PBMCs exosomal miR-155 and M1 macrophage polarization.
