| Objective:In order to develop innovative approaches for the clinical treatment of vascular dementia(VD),it is important to monitor the clinical effectiveness of acupunctur efor well differentiation,eliminating blood stasis,and clearing collateral-collateral acupuncture mixed with moxibustion,so as to provide a new method for the clinical treatment of VD patients,and to explore the effects of the combined treatment of acupuncture and moxibustion on cholinergic energy of VD patients.Methods:Patients admitted to the Department of Massage,Anhui Hospital of Traditional Chinese Medicine’s inpatient Department of Neurology and outpatient Department from August2021 to December 2022 were collected.Patients meeting the inclusion criteria were randomly divided into two groups,with 15 cases in each group.On the basis of controlling the underlying diseases,the control group was given oral western medicine,Donepezil 5mg daily,4 weeks as a course of treatment;In the acupuncture and moxibustion group,after treatment based on syndrome differentiation,acupuncture was performed at corresponding well points on the basis of routine acupoint selection,and moxibustion at three points of Dazhui,Shenting and Baihui was performed once every other day for 4 weeks as a course of treatment.Both groups were treated for 6 courses.Prior to treatment,three months after treatment,and six months following treatment,the Simple Mental State Inventory(MMSE)and Montréal Cognitive Assessment Scale(Mo CA)were found.Extracts of acetylcholine(Ach)and acetylcho-linesterase from serum of patients were also detected.Ach E)to evaluate clinical efficacy.Results:(1)There were 12 patients in each group of the 24 who met the criteria.Before therapy,there were no discernible differences in age,gender,disease severity,or educational attainment(P > 0.05).(2)The intra-group comparison results of MMSE,Mo CA,Ach and Ach E groups were before treatment,3 months after treatment and 6 months after treatment,P<0.05,with the increase of treatment course,the indicators of patients were significantly better than before treatment,indicating that for the treatment of vascular dementia patients,its clinical effect is positively correlated with the treatment course.(3)MMSE and Mo CA scores were increased,Ach content was increased and Ach E content was decreased in acupuncture and moxibustion group and western medicine group before treatment,3 months after treatment and 6 months after treatment,P<0.05,the total effective rate of the acupuncture group was 83.33%,whereas the total effective rate of the western medicine group was 66.67%,showing a substantial difference that both groups were equally effective,but the acupuncture group was superior to the western medicine group.Conclusion:(1)Acupuncture and moxibustion combined treatment of VD and oral Donepezil hydrochloride dispersive tablets can improve the cognitive function and executive ability of patients with VD.(2)The treatment course of VD patients was positively correlated with the improvement of cognitive function.(3)The MMSE and Mo CA scores of VD in the combined treatment of acupuncture and moxibustion were higher than those of oral Donepezil hydrochloride dispersive tablets,the total effective rate was clearly higher than that of the group using western medicine because the serum index Ach climbed more significantly and the serum index Ach E fell more dramatically.Meridian differentiation,acupuncture,and moxibustion are more effective clinically than oral Donepezil hydrochloride dispersive at removing blood clots and dredging collaterals..(4)Acupuncture and moxibustion combined with VD can protect the activity of cholinergic neurons,reduce the degradation of acetylcholine,and thus increase the signal conduction of cholinergic pathway,and the effect is better than that of western medicine group. |
