ObjectiveTo observe the clinical efficacy of head-body acupuncture with acupuncture points in the treatment of hemifacial spasm, and to clarify the advantages of head-body acupuncture with acupuncture points in the treatment of primary hemifacial spasm by comparing the efficacy with that of the conventional acupuncture methods,so as to provide an idea and method for the clinical treatment of primary hemifacial spasm.MethodSixty cases meeting the diagnostic criteria, inclusion criteria and exclusion criteria of hemifacial spasm were selected from the outpatient clinic of the Third Department of Acupuncture and Moxibustion of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine and randomly divided into 30 cases each in the treatment group(head and body acupuncture with acupuncture points)and the control group(conventional acupuncture method)according to the random number table method. In the treatment group, the main acupuncture points were selected as follows: head acupuncture: 2/5 below the anterior temporal oblique line on the healthy side;body acupuncture: Yin Tang,Tai Yang,Yi Feng,Feng Chi,Neiguan,Shen Men,He Gu and Taichong;according to the principle of proximal acupuncture points, the acupuncture points of the spastic area were selected for treatment. In the control group, conventional acupuncture was used for treatment.Patients in both groups were treated with acupuncture once a day for 50 minutes each time, with a break of 1 day after 6 consecutive days of treatment, and 7 days as a course of treatment, for 4 consecutive courses of treatment. The degree of spasticity and overall efficacy of the treatment and control groups were assessed using the Penn Spasticity Frequency Rating, the Cohen Spasticity Intensity Rating and the SAS score before treatment and at the end of the 4 sessions respectively.Results1.Comparison of basic dataThe basic data such as gender, age and duration of disease were not statistically significant((49)>0.05)when comparing the two groups of patients, and were comparable.2.Comparison of Cohen’s spasticity intensity classificationComparing the Cohen spasticity intensity grading between the two groups before treatment, the result was: Z =-0.682,(49)>0.05,no statistical difference, comparable. After 4 courses of treatment, the Cohen spasticity intensity was compared between the two groups before and after treatment.Intra-group comparison: treatment group Z =-4.282,(49)<0.05;control groupZ=-2.402,(49)<0.05 and inter-group comparison: Z=-2.672,(49)<0.05,statistically significant.3.Comparison of Penn spasticity frequency gradingComparison of the Penn spasticity frequency grading between the two groups before treatment yielded: Z=-0.233,(49)>0.05,no statistical difference, comparable. After 4 courses of treatment, compare the intensity of Penn spasm between the two groups before and after treatment.Within-group comparison: treatment groupZ=-4.824,(49)<0.05;control groupZ=-4.379,(49)<0.05,between-group comparison: Z=-2.164,(49)<0.05,statistically significant.4.Comparison of SAS scoresComparing the SAS scores of the two groups before treatment yielded:t=-0.219,(49)>0.05,no statistical difference, comparable. After four courses of treatment, the SAS scores of the two groups were compared before and after treatment.Intra-group comparison: t=17.979,(49)<0.05 for the treatment group;t=12.064,(49)<0.05 for the control group, and inter-group comparison: t=-3.097,(49)<0.05,which is statistically significant.5.Evaluation of therapeutic effectAfter 4 courses of treatment, the effective rate of the treatment group reached 90.00% and the effective rate of the control group reached 73.33%,the difference was statistically significant((49)<0.05),indicating that the efficacy of the treatment group was significantly better than that of the control group.Conclusion1.Both treatment protocols were effective in improving the spasticity intensity and spasticity frequency of hemifacial spasm, and the efficacy of the head-body acupuncture method with acupuncture points was significantly better than that of conventional acupuncture in treating facial myospasm.2.Both treatment protocols were effective in improving the anxiety state of patients with hemifacial spasm, and the head-body acupuncture with acupuncture was significantly more effective than conventional acupuncture in improving the anxiety state.
