Objective:To explore the clinical effect of low frequency acupoint electrical stimulation on the reconstruction of hand function after stroke by observing the effect of low frequency acupoint electrical stimulation on the surface EMG signal of thumb-to-finger movement related muscle after stroke,and optimize the treatment plan.Methods:60 patients who met the inclusion criteria were randomly divided into low frequency group(n=3 0)and electroacupuncture group(n=30).Both groups were given routine treatment of acupuncture and moxibu sti on.In the electroacupuncture group,the acupoints were selected as follows:Shuigou(DU26),Neiguan(PC6),Jiquan(HT1),Chize(LU 5),Jianyu(LI15),Shousanli(LI 10),Hegu(LI4),the waveform was intermittent wave,the frequency was 2Hz,and the stimulation intensity was the degree of patient tolerance.In the low frequency group,the acupoints were selected as follows:Shousanli(LI10),Waiguan(SJ5),Neiguan(PC6),Ximen(PC4),Yuji(LU 10),Hegu(LI4),the waveform was intermittent wave,intermittent ratio was 1:2,pulse width 0.3ms,frequency was 50Hz,and the stimulation intensity was based on patient tolerance.Both groups were treated for 30 minutes each time,once a day,6 days a week and 1 day off for 3 weeks.Surface electromyography(sEMG)eigenvalues(MAX and RMS),Chinese stroke scale(CSS),Brunnstrom motor function evaluation,modified Ashworth scale(MAS),Lindmark hand function score and Lovett muscle strength grade were used to evaluate before and after treatment.The clinical trial data collected were statistically analyzed by SPSS26.0 statistical software.Results:1.sEMG eigenvalues(MAX and RMS):After treatment,the sEMG eigenvalues(MAX and RMS)of the first interosseous dorsal muscle and abductor pollicis brevis in the two groups were increased,and the increase in the low frequency group was significantly better than that in the electroacupuncture group(P<0.05).2.CSS score:After treatment,the CSS score of the two groups decreased,and the decrease in the low frequency group was significantly better than that in the electroacupuncture group(P<0.05).3.Brunnstrom motor function score:After treatment,Brunnstrom motor function score increased in both groups,and the increase in the low frequency group was significantly better than that in the electroacupuncture group(P<0.05).4.MAS grading analysis:After treatment,MAS grading decreased in both groups,and the decrease in the low frequency group was significantly better than that in the electroacupuncture group(P<0.05).5.Analysis of Lindmark hand function score:After treatment,Lindmark hand function score increased in both groups,and the increase in the low frequency group was significantly better than that in the electroacupuncture group(P<0.05).6.Analysis of the results of Lovett muscle strength grade:After treatment,the Lovett muscle strength grade of the two groups increased,and the increase in the low frequency group was significantly better than that in the electroacupuncture group(P<0.05).7.Effective rate statistics:There were 28 cases in the low frequency group,7 cases were cured,17 cases were significantly improved,2 cases were improved,2 cases were ineffective,and the total effective rate was 92.86%.In the electroacupuncture group,2 9 cases were cured,13 cases were significantly improved,6 cases were improved,6 cases were ineffective,and the total effective rate was 79.31%.There was significant difference between the two groups(P<0.05).Conclusion:1.Both low frequency acupoint electrical stimulation and electroacupuncture were effective in the reconstruction of motor function of thumb,which was embodied in the increase of MAX and RMS values of the first interosseous dorsal muscle and abductor pollicis brevis,the decrease of CSS score,the increase of Brunnstrom motor function score,the decrease of MAS grade,the increase of Lindmark hand function score and the increase of Lovett muscle strength grade,and the improvement of low frequency acupoint stimulation was more obvious.2.Low-frequency acupoint electrical stimulation therapy has a significant effect on improving thumb-to-finger movement of patients after stroke,and the curative effect is better than that of electroacupuncture,which is worthy of clinical application.
