Objective: To investigate the effect of preoperative neutrophil lymphocyte ratio and globulin on the prognosis of patients with upper urinary tract epithelial carcinoma.Methods: The data of 96 patients with UTUC admitted to the urology department of our hospital from January 2010 to November 2021 were collected retrospectively,and the working characteristic curve(ROC)analysis was used to determine the optimal cutoff value of neutrophil lymphocyte ratio and globulin,and the G-NLR score was 0 when the ratio of globulin and neutrophil lymphocyte was lower than its optimal cutoff value;When only one item is higher than the optimal cutoff value,the G-NLR score is 1 point;When both are higher than the optimal cutoff value,the G-NLR score is 2 points;Group according to the scoring results and compare the baseline data.Total survival rate(OS)is defined as the time from the beginning of surgery to death due to any reason,and tumor-specific survival rate(CSS)is defined as the time from the beginning of surgery to death due to cancer;In OS and CSS analysis,Kaplan-Meier curve was used to evaluate the survival rate of different groups,and Log-Rank test was used to test the survival curve.In addition,logistic regression analysis was used to identify independent risk factors.Results: In univariate analysis,age,tumor grade,lymphatic vessel invasion,tumor stage,tumor size,tumor recurrence or not,and G-NLR score were significantly correlated with patient OS;Gender,tumor grade,tumor stage,tumor size,outcome events,G-NLR score were significantly correlated with patient CSS.Multivariate analysis showed that tumor grade,tumor stage and G-NLR score were independent risk factors for OS and CSS in patients with upper urinary tract epithelial carcinoma.Kaplan-Meier survival analysis showed that a high G-NLR score suggested poor OS and short CSS.Conclusion:Preoperative G-NLR score is an independent risk factor for the prognosis of patients with upper urinary tract urothelial carcinoma.A higher G-NLR score indicates worse OS and worse CSS,and the acquisition method is simple,economical and applicable.It can become a biomarker that can provide effective help for individualized treatment of malignant tumors and postoperative follow-up,and is worthy of clinical application.
