Objective: To explore the level changes and clinical significance of serum amyloid A(Serum Amyloid A Protein SAA),cerebrospinal fluid SAA,and C-reactive protein(C-reactive protein,CRP)in adult patients with viral encephalitis. Methods: Selected 50 patients from December 2021 to December 2022,50 cases,58 patients were hospitalized as the control group, using serum and cerebrospinal fluid SAA immunoassay, immune turbidimetry to detect CRP level, according to the clinical data, symptoms and auxiliary examination results of the patients into mild group and severe group. At the time of discharge, the patients were divided into good prognosis group and poor prognosis group according to the sick Glasgow(GOS)prognosis score scale, and the difference between the study indicators in the control group and observation group and observation group was further analyzed, data was processed using SPSS 25.0 software, and P <0.05 was considered to be statistically significant. Results: The serum SAA,serum CRP,and CSF SAA levels of the viral encephalitis patient group were all higher than those of the control group,Among these, only serum SAA was statistically significant between the two groups(P <0.05);Serum SAA,serum CRP,and CSF SAA were all higher in the severe group than in the mild group,The serum SAA and CRP were statistically significantly different between the two groups(P <0.05),CSF SAA was not significantly different between the two groups(P> 0.05);The level of the three indicators in the poor prognosis group was higher than that of the good prognosis group,The difference was statistically significant(P <0.05);The ROC area under the ROC curve of serum SAA,serum CRP and cerebrospinal fluid SAA index predicting poor prognosis of viral encephalitis were0.9487,0.7043,06306,respectively,Sensitivity of 0.846,0.769,and 0.308,Specifically were 0.917,0.604,and 0.958,The best threshold was 42.28,2.015,and 0.045,It suggested that serum SAA had a high accuracy in predicting the poor prognosis of viral encephalitis. The levels of the three inflammatory indicators decreased significantly compared with before treatment, with the difference between serum SAA and serum CRP(P <0.05)and not before and after SAA in CSF(P> 0.05).Conclusion: The serum SAA index level changed significantly after infection, which decreased greatly after treatment. The combined detection of serum SAA and serum CRP is of certain value in distinguishing mild disease from severe viral encephalitis. In addition, the accuracy of serum SAA in predicting poor prognosis is better than that of serum CRP and cerebrospinal fluid SAA,and clinicians can make a preliminary judgment on the situation of patients with viral encephalitis based on the results of serum SAA and serum CRP test.
