Objective: To develop a specific scale of readiness for discharge of neonates with enterostomy with good reliability, with the aim of providing a scientific and practical tool for assessing readiness for discharge of neonatal caregivers with enterostomy.Methods:1.A systematic literature search was conducted through the evidence-based "6S" evidence resource pyramid model, combined with qualitative semi-structured interviews, and group discussions to form an initial pool of entries on discharge readiness of neonatal caregivers with enterostomies.2.The importance and content of the scale items were evaluated through two rounds of Delphi expert consultation to form the initial readiness for discharge scale for neonates with enterostomy.3.Using convenience sampling,a total of 182 enterostomy neonatal caregivers were surveyed using the Initial Readiness for Discharge Readiness of Enterostomy Neonatal Discharge Scales from April 2022 to December 2022.The entries of the scale were further screened using the correlation coefficient method, the critical ratio method and the factor analysis method; The validity test of the scale selects the validity of the content and the validity of the structure for testing;Cronbach’s α coefficient and halving coefficient were selected to test the reliability of the scale, and finally formed the enterostomy neonatal discharge readiness scale.Results:1.In this study, after an evidence-based literature search of 6 articles, qualitative semi-structured interviews with 12 caregivers of neonates with enterostomies, and iterative discussions among the subject members, the initial entry pool of the Readiness for Hospital Discharge Scale for Neonates with Enterostomy was formed to contain a total of 26 entries in 4 dimensions: child and caregiver status(5 items),nursing knowledge(8 items),care ability(7 items),and expected support(6 items).2.Based on the results of two rounds of expert consultation, the initial scale with 4dimensions and 28 items was finally formed.3.The results of project analysis showed that there were no deleted items, and the CR values of all items were greater than 3,and the differences were statistically significant(P<0.001).The correlation coefficient method was used to calculate the correlation coefficient between the 28 items of the scale and the total score of each scale, and the results showed that the correlation coefficient of each item was greater than 0.3,and the difference was statistically significant(P <0.001)。Content validity: The I-CVI of this scale ranged from 0.786 to 1.00,and the S-CVI/Ave was greater than 0.9,indicating that all the experts consulted agreed with the content of this scale, and the content validity of the prepared scale was high.Structural validity: after three exploratory factor analysises extracted four common factors, the final cumulative variance contribution value was 71.18%,the scale entries had low loading coefficients on other factors, only the loading coefficients on the corresponding factors were greater than 0.4,and the structural validity was good.Reliability tests: 0.947 is the Cronbach’s alpha coefficient for the total scale,0.872,0.941,0.867,and 0.884 are the Cronbach’s alpha coefficients for the four dimensions,respectively;0.895 is the total fold reliability coefficient,0.836,0.925,0.744,and 0.895 is the total half reliability coefficient,0.836,0.925,0.744,0.885 are the half reliability coefficients of each factor respectively, all of which meet the reliability test criteria, indicating that the scale has good internal consistency. Conclusion:This study followed the standardized process of scale development, and the Enterostomy Neonatal Discharge Readiness Scale was developed with good reliability and validity, which can be used as a valid tool to evaluate the level of discharge readiness of enterostomy neonatal caregivers in China and provide some reference for the development of individualized discharge planning interventions for enterostomy neonatal caregivers.
