| Objectives1.To construct a scientific,reasonable and suitable evaluation index system for the post competency of CRRT nurses.2.According to the constructed post competency evaluation index system,the post competency self-assessment questionnaire for CRRT nurses was formed to investigate the current status of post competency of nurses engaged in CRRT.Methods1.Using the literature retrieval method and "competency" as the theoretical framework,through literature review,the post training of nurses engaged in CRRT in tertiary hospitals and the research status of competency evaluation were understood,and the frame of the post competency evaluation index system was sorted out.2.Developed an interview outline,combined with semi-structured interviews and behavioral event interviews,qualitative interviews were conducted with 11 nurses,3nursing managers and 2 doctors engaged in CRRT to understand the job responsibilities and working effects of CRRT nurses.To summarize the characteristics and specific content of continuous renal replacement therapy nurses’ post competency.According to the interview content,the theme was sorted out and refined,and the first draft of the post competency evaluation index system for CRRT nurses was formed combined with the results of literature review.3.The expert consultation method was used to select experts in the fields of nursing management,nursing research,clinical nursing,nursing education and so on.Two rounds of consultation were conducted.4.The analytic hierarchy process was used to construct the hierarchical structure model of the evaluation index,and the weight of each evaluation index was established.5.Using the questionnaire survey method,a self-assessment questionnaire for continuous renal replacement therapy nurses’ post competency was developed on the basis of the evaluation index system.From September 2022 to November 2022,clinical nurses from tertiary general hospitals in different regions of China were selected by convenience sampling method to conduct the survey,and item analysis and reliability and validity test were used.The evaluation index system was verified.Results1.Three themes and 14 sub-themes of competency characteristics of CRRT nurses were extracted through qualitative interviews.2.A total of 18 experts were selected from 7 tertiary general hospitals in Beijing,Shandong,Shaanxi and Hubei for two rounds of Delphi consultation.The authority coefficients of the two rounds of consultation were respectively 0.875 and 0.894,the coefficients of variation were respectively 0~0.31 and 0~0.24,and the Kendall’s W coefficients were respectively 0.190~0.342 and 0.184~0.369(all P<0.05).Finally,the evaluation index system of CRRT nurses’ post competency was constructed,including 3first-level indicators,9 second-level indicators and 47 third-level indicators.3.3 The weight of knowledge,skills,attitude and values were 0.25,0.50 and 0.25,respectively.The weight of 9 second-level indicators ranged from 0.0186~0.3333,and the weight of 47 third-level indicators ranged from 0.0012~0.0822.4.The post competency of 368 CRRT nurses was investigated,and the reliability and validity of the evaluation index system were tested.The CR value was 9.81~26.70,all CR > 3.00,and P<0.001;Correlation coefficient was 0.549~0.905(P<0.001);KMO value was 0.979;The Cronbach’s a coefficient was 0.989,and the split-half reliability was 0.958.5.The score of post competency of CRRT nurses was 55~235,with an average of178.01.The age of CRRT nurses was 21 ~50 years old,and the median working years in the nursing field was 8 years,and the median working years in CRRT field was 5 years.Bachelor degree accounted for 92.1%;Primary titles accounted for 62.2%.The results showed that there were differences in the post competency scores of nurses in terms of gender,marital status,work department,professional title,position,monthly bonus amount,whether they obtained provincial or above blood purification professional certificate,and whether they participated in continuous renal replacement therapy related training organized by other hospitals(all P<0.05).Age,working years in the nursing field,working years in the continuous renal replacement therapy field were positively correlated with the post competency score of nurses(all P<0.05).Conclusions1.The evaluation index system of CRRT nurses’ post competency constructed in this study is conducive to the evaluation of CRRT nurses’ post competency from the overall perspective,and provides a reasonable and effective evaluation basis for the employment,training,performance appraisal,performance distribution,reward evaluation and other work of nursing staff.2.The structure of CRRT nurses in China is low in seniority,low in professional title,and few with high education,and their post competency needs to be improved. |
